Is it a good sign when Social Security Disability sends you to a doctor?I was that doctor, doing exams for Social Security as a part-time activity for a decade or so, and I also had a medical practice in which some of my patients might be applying for Social Security Disability.Being sent to a doctor for an exam is not a “good sign” or a “bad sign.” It is what happens if the information that the doctors and other decision-makers at Social Security looked at was inadequate for determining whether or not you qualify.Your information might be inadequate because you don't see doctors. One person I evaluated was clearly dying and would have had no problem qualifying for immediate acceptance, if there had been a medical record to review. I wrote up the case emphasizing that eligibility. A few weeks later, I was contacted by the coroner to certify that the death didn't require an autopsy, because there was no other doctor to ask.Many of the cases I examined had medical records focusing on diagnosis and treatment but with little or no detailed information on strength, mobility, or other factors that would affect ability to work. The Social Security Disability exam has a very different perspective from the usual clinic visit. In my own practice, I was aware of what Social Security would need for making a decision, and I made sure to include that information in my notes. Not every doctor does that.Some cases are sent for examination because of reports to Social Security that the person might be making a fraudulent claim. An ordinary medical record rarely contains the information that a reviewer needs to determine whether this is likely. Both in my Social Security exams and in writing clinic notes for my own patients, I recorded my observations supporting or not supporting the patients’ claims.Some conditions are not addressed in Social Security's guidance for determining disability. I did one exam for excessive sweating and was able to report that the patient (who was otherwise in reasonable physical and mental condition) left a large pool of sweat on the exam table and needed to drink liquids during the exam to keep up with the water and electrolyte losses.Some exams are required in Social Security regulations, even if they seem ridiculous. I stopped doing Social Security exams and then retired before this plague was really bad, but I wouldn't be surprised if it is causing more and more problems. That's “templating.” Electronic records make it easy to record all the parts of the physical exam that are required for payment from the insurer by just importing a description of the routine exam (the stuff that is just incidental to whatever the doctor was actually looking at, but which needs to be mentioned anyway). I have seen the eye exam reported as normal on a patient with a glass eye, and many cases of joint problems simply not noticed in templated notes.So don't take it as a good or bad sign if you are required to go for a Social Security Disability exam. Most of the reasons for requiring those exams don't have much to do with you, but have a lot to do with how medical records are kept.
