The main problem in the US is that the system of widespread private insurance, with different networks and copays and so on, is enormously complicated, leading to a lot of time being wasted on paperwork, and huge amounts of money going to support the firms that generate the paperwork.Then, universal healthcare is prioritised by urgency, not ability to pay. Also, as Michael points out, the fact that private hospitals are used to speed up non-urgent treatment, and for mental health for which the NHS is over-stretched, and for purely cosmetic surgery which the NHS doesn’t cover, means that they don’t need to maintain facilities for really serious/urgent cases, and not having to have an A&E or an ICU means they can keep their costs, and prices, much lower than in the US. Only the kind of private hospital that caters for royalty and Russian oligarchs can afford to, or needs to, have an ICU.Also, the US system makes it impossible for hospitals or insurers to negotiate good price deals with the big pharma companies, and it obliges the pharma companies to charge high prices because they are paying a huge amount for advertising. Under universal healthcare (except in New Zealand for some reason), the pharma companies advertise prescription medications only to doctors and health boards, slashing their advertising costs, and the health service is able to make huge economies of scale, by negotiating good deals for bulk-buying drugs and equipment.
