As a therapist, what is the most important lesson a patient has taught you?

As an addiction therapist who treats not only substance abuse, but specializes in treating sex addiction, the most important lesson a client has taught me is to remember we are all fallible, flawed and we’re only human. This is applicable on several levels.I had a client I will call Nicole. She was a beautiful girl, a nurse, who had recently moved to North Carolina to escape her abusive boyfriend and start a new life closer to family.Her boyfriend would convince her to use alcohol, coke and heroin to “enhance” their sex life. They’d get high and have sex in risky places. Nicole found this demeaning and shameful but she wanted to please her boyfriend.When Nicole used certain substances, like alcohol, she became very angry and violent, often getting into fist fights with her boyfriend who would call the police and have her arrested. He beat and raped her too, but she believed she deserved it, so she never reported him. She just wound up in the hospital multiple times.Nicole came to see me becuase she knew she needed help. She was addicted to drugs, alcohol, and, not only sex, but her arousal template[1][1][1][1], which includes using substances, and the violence and shame leading to sex.Throughout our work together, Nicole shared she had been sexually abused as a child. She had never told a soul, living with that shame and the belief she was damaged, would never been anything but trash and unworthy of love or kindness. The trauma had created maladaptive core beliefs for Nicole, the beliefs we hold about ourselves. They shape who we are and often inform our subconscious and conscious choices.After a few months, Nicole disappeared. She called me from Florida outside a restaurant crying. Her ex was in the restaurant and she wanted to go inside, but knew she could get hurt and be repeating her addiction cycle[2][2][2][2] if she did.I advised her to remember all the work she had done and to use the tools she had been given, trying to detemine if she was going to harm herself or someone else, all the time thinking, “This girl is going to get into real trouble, I’m scared for her life, and what is she doing in Florida?!?”The next call I received was from her mother. Nicole had been arrested and charged (again) with domestic violence. At least she was alive.Nicole went to treatment in Florida as part of her legal agreement. I never heard from her again until I googled her out of curiosity.I found her obituary. She had died of an overdose.What did I learn from Nicole?I can not fix people.It is not my job to heal or “fix” my clients. It is my job to guide and educate them, providing them with the tools they need to heal themselves.I can not save them or “make” them change. They have to do the work themselves.Stigmas around addiction are real.This was a beautiful, smart young woman from an upper-middle class family. Her parents had been married for 30+ years. She had healthy, happy siblings, lots of friends. This was the homecoming queen and head cheerleader. The valedictorian and soccer star.Before I understood sex addiction, I believed sex addicts were all male, pedophiles, sex offenders and rapists who were flashers and camped out in parks. This could not be further from the truth.Most sex addicts are smart, successful, wealthy people. By the way, many drug and alcohol addicts are your doctors, attorneys, executives, CEOs and your “nice 80-year-old next door neighbor.”If you believe you don’t know an addict, you’re probably in denial or you don’t know you know one.Addiction isn’t curable.There is no cure for addiction. It is a lifelong disease which can be in remission, but not cured. We don’t ask people who are diabetic to “just stop” taking their insulin. We don’t call people with cancer, “cancerous.” We don’t ask people in support groups for cancer, “How long they’ll have to go to those meetings?”If we can begin to look at addiction as a biopsychosocial disease, we can learn to be more empathetic and compassionate.Nicole would always have to maintain her recovery and address and think about her addictions. That’s what it means to be in long-term recovery.We all have our s**t.For me, I know I can be codependent. My instinct is to want to fix people. It’s a toxic quality that I’ve worked for years to grow awareness and coping mechanisms around. I am also a recovering perfectionist. I believed, for most of my life, that if I could keep everything bad hidden and bottled up from the world, everyone would only see the happy, pretty, perfect wife, friend, family member and mother. That didn’t work out too well. It took a lot of my own therapy to accept myself for who I am, be authentic and vulnerable with people, change my own core beliefs and believe I am worthy of love and kindness.Everyone has their s**t. At least some of us, like my clients, are aware of ours and trying to do something about it.I am human and I will not have all the answers.For months after Nicole died I asked myself, “What else could I have done? What if I had X? Why didn’t I Y?” The reality is, I did everything I believed I could do to provide her with the care she deserved from me. In hindsight did I have other ideas of things I could have tried?
