Living Will Questionnaire Form: Fill & Download for Free

When I was diagnosed with rectal cancer, I went through the Advance Directive/Living Will questionnaire at LegalZoom (not endorsing them, that’s just what I used).It’s full of questions like “If I am unconscious and it is very unlikely that I will ever become conscious again,” do I want tube feeding? Are these supposed to be trick questions? Why would you want food if you’re permanently unconscious? Does anyone want that?Then I realized that “yes” is the default for people without living wills. It’s been over 10 years since I’ve done any hospital medicine. I forgot how messed up end-of-life care can be.To answer your question: if I’m not living, let me be dead. If I want a feeding tube, I will ask for it. If I want long-term ventilation, I will signal my consent. If, after a trial period, it’s unlikely I will ever gain the ability to positively consent, pull the plug.

Yesterday. I had a doctors appointment, and I guess it had become a thing for them to ask you about your mental as well as physical health now… they specifically had a “fill in the bubble” kind of questionnaire which asked about depression, suicidal thoughs, emotional stresses of various kinds. They also had check-boxes for whether you had a living will or “advanced directives” paperwork on file.The irony is I was happier walking in and more depressed after filing out the form…

I started grad school in psychology in 1986. I got my Master's degree in developmental psychology in 1989 and my Ph.D. in neuropsychology in 1991. Over that time, I was busier than any other time I'd been in school. Graduate study is completely different than undergraduate life. You start with a focus on a particular aspect of your field - in this case, psychology, and you spend the next 2 to 5 years intensifying that focus until you and your master professor decide on a topic of research.Your master professor is the person you will be working with while you're a grad student. They act as a mentor, a critic of your work, and are your most important resource about the field you've chosen to study. You meet with them often, discussing articles from academic journals and how they apply to the work you professor (and, hence, YOU) are researching. Expect a lot of reading, especially of articles from peer-reviewed journals. These will be a big part of most of your graduate classes, and the foundation on which you will build your Master's thesis and, if you go further, your Ph.D.Be prepared for a lot of reading and a lot of writing - research papers, midterm and finals papers, and, if your lucky, articles that go on to be published in respected journals. Depending on your interests in psychology, you could be up late at night in a lab, helping to conduct a study. You could be working with people answering questionnaires or participating in experiments that you and your major professor design. You'll probably get some experience with teaching classes, first as a teaching assistant and then, possibly as a real instructor for a basic undergraduate course.You'll be exhausted most of the time, you won't have much money, and the places you live will most likely be small apartments or houses. You'll work with other grad students who share the same major professor, and, through early morning meetings and late nights in the lab, you'll probably find a good friend or two, three, maybe more. They'll help you celebrate all the milestones you reach and are the people in your life who can best understand what a crazy life you're living.And after all that, you'll have a degree that means not only that you've mastered a topic within psychology, but that you're competent to set goals, meet deadlines, and create something totally new all on your own.
