How should a physician practice transfer data from paper records to EHR?

Certain medical societies, such as the California Medical Association, recommend that all medical records be retained indefinitely. If you plan to move from paper charts to an EMR/EHR, put your existing paper charts into offsite storage, and re-purpose office space currently used for your chart room, then I suggest the following:Convert all paper charts to PDF format, one PDF per patient chart. Metatag the PDF with PII. We've found that scanning at 1200dpi, monochrome, then encoding with JBIG2 lossless compression, provides appropriate resolution and reproduction quality.Convert static images -- X-Rays, CT scans, etc. -- to a lossless digital format. DICOM supports embedding PII in the image metatag.Convert multimedia -- recordings of heart sounds, ultrasound exams, angiograms, etc. -- to a digital format.Abstract key clinical information from each chart into your patient registry (your EMR/EHR). As a starting point, you could use the CMMS MU guidelines to determine the minimum data set necessary for clinical abstraction, adding additional information you may consider clinically relevant, such as TNM and tumor marker information, to your abstracting process.Steps 1-3 can be done by existing front office employees, or by a subcontractor after execution of a business associate agreement. The digital documents created in steps 1-3 should be attached to the patient record in the EMR, so that you can refer back to the electronic image of the originating non-digital document.Step 4 requires a clinically-trained individual who understands clinical shorthand and the difference between a problem list entry, a clinical finding, a chief complaint, an assessment, a differential diagnosis, a pre-authorization, a formulary change, etc. We've had success automating this step using clinical ontology mapping and NLP methods combined with expert review. Consider the challenges a non-clinical individual would face abstracting the following: Mr. N is a 52 y/o mildly obese AAM w/ DM2, HTN, CAD, S/P CABGx3, currently on statin and ASA, who presents w/ atypical CP, 160 SBP, cardiac enzymes, cholesterol panel and LFT WNL, no significant findings on PEx and CXR, and non-specific ST changes on EKG.Offshore data entry is not an option.Costs for the process I've outlined above can range from $10,000 to over $50,000 for the average primary care physician with the average practice panel. There are a number of fine points to this process which are beyond the scope of Quora. Feel free to contact me directly with any questions.

What are the major hurdles in adoption of data science in healthcare?

The government and too many regulations. Healthcare is a very regulated industry with tons of rules and regulations. It would be nice to have a big central database of every visit, lab test and prescription you ever had, but it cant be done due to the HIPPA privacy laws, where your information can't be shares between institutions without your express permission. So right now no institution can talk to each other. Since they can't communicate freely, there is no one standard EMR (Electronic Medical Record) and every system has a program that is unique to themselves. An analogy would be if one hospital had windows computers, another had Apple, another ran Linux, and another ran only off an ENIAC mainframe. Now try them to run the same program! They can all make a printout of their data and send it around, but unless it is reformatted into the other institutions language, its just a pile of dead paper sitting in the corner. That is literally the case, if I get records from an outside hospital, it is scanned into the patient's chart as a PDF file that I can read, but it won't do anything useful and I can't crunch numbers with the data! The other problem is that because an EMR is such a big purchase, the high ups are involved in the purchase of the system. These people are administrators or doctors who have become administrators and don't see patients on a day to day basis. So the systems that are being purchased look impressive, but are a poor fit for the people who use it day to day. Right now healthcare data science is at least 10 years behind in technology and shows no signs of catching up.

Are other governments like China censoring the impact of coronavirus?

You can not trust any communist government since is prime concern is not to do what is right but to exalt itself as the superior form of governance, out of lust for power.
