Yes, I have. I’m 37. Needless to say, when I spoke with my physician about it, she definitely raised her eyebrows and questioned me a bit. Once she heard my reasoning, she had no problem signing off on the order. Here’s why I chose to have a DNR:I’ve been a paramedic for over seventeen years. I’ve personally seen more death and witnessed the process more than most entire families will. I believe I have a better understanding of death and dying than most Americans. So many people I’ve spoken with seem to have obtained their understanding of death from Hollywood. They think they’ll drift peacefully off to sleep after saying goodbye to family, and then just never wake up. Let me assure you, it almost never happens that way. For the vast majority of people, life ends with discomfort and pain, or struggling to breathe, and typically at the end of a long, drawn-out disease process.On top of that, most people don’t realize that there’s a difference between “clinical save,” and “survival to hospital discharge.” A clinical save is what you’ll typically see portrayed on TV, where the Doctors, nurses, or medics “get the heart going again.” A clinical save is when you achieve return of circulation and the patient survives long enough to be admitted to the ICU.But here’s the dirty “secret:” after arriving at the ICU, it doesn’t matter if the patient dies three minutes after he or she arrives. It still “counts” as a “clinical save.”Now, survival to hospital discharge is exactly what it sounds like: Did the patient live to be discharged from the hospital? What many people don’t realize, and many doctors don’t seem to want to acknowledge is, that there’s a HUGE difference in success rates for the two categories. Approximately 40–50% of cardiac arrests result in a “clinical save” Sounds good, right? Here’s the catch: survival to hospital discharge rates are (at the highest, at the time I’m writing this) 14%.And the news gets worse. Most of those 14% are people who suffered cardiac arrest while already inside a hospital. The survival to hospital discharge rate for cardiac arrests that occur outside the hospital are in the mid-single digits. On top of that, half of all “survival to discharges” are NOT neurologically intact. Those unfortunate people are discharged from the hospital straight to nursing homes, where they’ll live out the rest of their existence (which goes back to my point about drawn-out disease processes and death). The other half survive with varying levels of neurological function ranging from significantly impaired to little impairment.In my 17 years of working EMS, I’ve worked dozens, if not hundreds of cardiac arrests. About half have been “clinical saves.” The number that have survived to hospital discharge: one.I’m enjoying life. I hope to continue to enjoy a decently long life, and help as many people during a medical emergency as I can along the way. But I’ve always been a quality-rather-than-quantity of life type of person. Under NO circumstances do I EVER want or consent to permanently performing bodily functions through a tube or machine. I simply decided that the current state of medicine does not provide good enough odds in the event of cardiac arrest. If improvements are made, I’ll re-evaluate my decision.
