Is only one cause of death put on a death certificate? If so, might COVID-19 deaths be over/under reported?

This answer may contain sensitive images. Click on an image to unblur it.The standard U. S. death certificate has up to 4 causes of death. The first is the immediate cause of death, and the fourth is the underlying cause of death as shown on the following blank form.Specialists called “nosologists” code the underlying cause of death for statistical purposes. There are also computer programs like, SuperMICAR[1] that aid in the process. Getting this right is extremely important for consistent statistical reporting. The immediate cause of death could be heart failure (the medical term for it), but that could be caused by electrocution, or Covid-19. If the person who died had Covid-19, and it was known, then it will be on the death certificate under one of the 4 causes. However, and particular in early cases, Covid-19 might not appear on the death certificate because it was not known. Some guesswork was involved before test kits were widely available.See also: Provisional Death Counts for Coronavirus Disease 2019 (COVID-19)Footnotes[1] NVSS - MMDS - SuperMICAR

As a doctor, what did you learn from your first patient death?

When I was a medical resident, I had seen my share of deaths in the ER, and the OR, and in the ICU. But none of these were “my” patients. I was part of a team, and the death was a “team” death. So although I felt sad and stressed about these deaths, I did not feel as though the death was “my” death, if that makes any sense. The first death that was “my” death occurred in a patient that was assigned to me in resident outpatient clinic. This patient identified me as his personal physician, although technically, I was still a trainee physician. One day, I was presented with a death certificate. The certificate was for this patient that I cared for in my clinic. It was my job to provide a “cause of death” for him on his death certificate. This was the first I had heard that my patient was actually deceased. I had seen him recently, and he had been fine.It turns out that he was found sitting in his chair at home. He had been watching TV and he simply passed away without warning. I looked through his chart. He had nothing to suggest any type of lethal illness. He had a couple of minor problems, nothing serious, and these problems were well controlled with medications. He was not that old. I had no idea what could have caused his death, but the most likely scenario was either stroke or sudden cardiac death.I had to write a cause of death, so I wrote, “sudden cardiac death.” But he did not have any known heart disease. For all I know, it could have been a hit. His wife could have poisoned him. His next of kin did not want an autopsy, so there is no way to know what actually caused his death. But the police did not think it looked suspicious, so I filled in the heart attack thing in the blank so they could legalize the document. I did not feel 100% comfortable doing that. I felt like I should really advocate for my patient, even though he was dead. But nobody wanted any further investigation. From his death, I learned that there is a frightening amount that we don’t know, and perhaps cannot know. I also suspect that there are a lot of people who have “sudden cardiac death” on their death certificates when nobody actually investigated how they died.

What do I do now that I see my husband's 1989 death certificate mentions "Infected generator for brain stimulator?"

What do I do now that I see my husband's 1989 death certificate mentions "Infected generator for brain stimulator?"Loss is a hard thing, and the years don’t make it go away. I am sorry.I gather there has been some questions about the true causes of your husband’s death. I wonder why it has been almost 30 years, and yet you are just now seeing that information? You should have been the first! You say the certificate “mentions” the infected generator. I would wonder in what capacity does in mention that.On a Death Certificate in Texas, there is a place for the Primary cause of Death,as a result of (blank), as a result of (blank).Without seeing the certificate, I would imagine that the infected generator might fall into one of those blanks for the underlying reason for the Primary cause of Death. But there is also a blank for the Doctor to list another condition, perhaps important, but not directly responsible for this death. Seeing the infected generator listed THERE would have a considerably different meaning. And of course, the Death Certificate where your husband died may be nothing like the one here in Texas. But HOW that infected generator is mentioned may be as important as the fact that it is mentioned at all.I wonder as well, what do you WANT to do now? Do you want to understand it better? After this much time, it might not be possible to talk to the doctors involved, they might have passed on themselves, or retired. But you can of course look for medical information, starting with what kind of stimulator he had, and where it was implanted. Also, where the generator was implanted. That might be different. My former wife had a “vagus nerve stimulator” (the vagus nerve is one of the 12 cranial nerves), and the generator sat high in her chest, like a pace-maker. It might help if you understood what his therapy was like, and what was involved. It sounds like something may have gone wrong with his treatment, an infection starting at the site where the generator was placed, perhaps. If this is so, you may be asking about how you should go about seeking justice for this thing going wrong. For that, you will need to consult a lawyer who does medical malpractice. But I would be rather surprised if there is anything to do after this gap of so many years. - as I said, the original Doctors are likely to no longer be practicing, and may even have passed away. Still, only a lawyer can properly evaluate that issue. My having doubts doesn’t amount to anything. If you are asking what you do now because your are finding yourself sort of “stuck” that is a good question. You may need a professional to help you get through that. If so, I would highly recommend a few sessions with a professional therapist to help things get back on track.I’m sorry for being so wordy, but you left us little to go on, in understanding where you are, and what you want. I tried to touch a little on the possibilities I can see.
