There was once when we were provided a predetermination letter that said it would be approved only to start treatment later and find out the benefit would not be paid.I think you should appeal the insurance company's decision to not approve the treatment. Oftentimes the member (you) can get the insurance company to reconsider their decision. I have patients that have done this and it works. Insurance companies are generally more receptive to members than providers (the dentist). In your situation, I think the dentist is the only person that could say whether you needed the deep cleaning. X-rays are 2 dimensional images that only provide a limited amount of diagnostic information to the dentist. The recommendation for the deep cleaning is more based on the intra-oral exam and specifically-periodontal pocket measurements. I have had a deep cleaning myself. After the cleaning, the xray showed minimal change, but my pocket measurements improved considerably.Keep the dentist in the loop and let them know you're trying to get this paid. They might be able to help you submit additional information to support your appeal. Even if your appeal doesn't work, maintaining a good relationship and keeping communication open will make the dentist more likely to offer you a reduced fee.Recently, I was able to convince my insurance company to pay for something they said they would reject too. If you look at publication 521 under " Moves to locations within the United States, on Page 8, there is an explicit warning that says:"You cannot deduct the cost of moving furniture you buy on the way to your new home."Implicit in this exclusion is the actual cost of buying furniture. As you can imagine, this would expose the IRS to fraud and abuse if they permitted such a deduction. You could try to ask the IRS for a predetermination letter but it's unlikely they would approve it because the intent is to allow a deduction for the costs unavoidably incurred in moving only. The other requirement is that the expenses must be reasonable and necessary which would be hard to prove when making a new purchase.The moving expense deduction is a very generous above-the-line AGI deduction meaning you don't even need to itemize to write it off. I would review those expenses that are allowed. Reasonable moving expenses may include the cost of gas or the mileage on your vehicle, rental trucks, short-term storage, and boxes. For a long move, you might include the cost of lodging at a hotel on the way to your new home. The IRS allows a standard mileage rate that you can use to calculate your travel expenses, but if you prefer, you can keep up with your actual transportation costs and deduct those instead. Eligible travel costs include gas, oil, parking fees, and tolls.
