This comment has two parts. The first is a physician's experience, posted on another website that I belong to, which I thought was very pertinent and important for this thread, and had over 100 comments from physicians who either sympathized or empathized. Below this account is my own list of reasons of why to avoid medicine unless you've absolutely certain you can handle these drawbacks. It is nice to be part of a community of people who can identify with what you are going through. I thought I would let you know the experience I have had over the last 3 years. Perhaps some will take heed or learn from my experience if there is anything to learn.I have been sued twice since the fall of 2007. One was completely bogus addition by a ass of a lawyer. I was let out of the case with just a few depositions and only a NOI, without an actual suit. The other had some merit, with the patient dieing in the hospital and the lawyer suing everyone involved trying to cash a lotto ticket for the family. 5 experts including the plaintiff expert agreed with me. I met the standard of care as agreed by 5 other blinded experts including a professor from the local university. They all agreed more or less with my handling of the case. And yet the suit persisted for 3 long years!I had an excellent lawyer who actually had me go to Georgia with him and sit across the table and eyeball the plaintiff while he questioned him. The plaintiff expert witness completely caved sitting in front of another physician, he essentially gave up the case. Recently a judge dismissed me with prejudice which means I cannot be sued again on this case. It is unfortunate that I no longer have to look up the terms "plaintiff" "NOI" "with prejudice" or a bunch of other legal terms that have become burned into my brain. Many of us has been sued but it is such a painful experience that we rarely talk about it amongst ourselves other than dark humor jokes. If I go over in my mind all the physicians I know, nearly all I know have been sued at one time or another. As you can imagine this really doesn't make it much better. I knew the possibility existed I just thought I would be able to handle it much better than I did. The 3 years were a harrowing experience to say the very least. When I first heard about it, I stayed in bed for nearly 2 weeks almost literally. I became so distraught and depressed that I seriously contemplated shooting myself as an act of defiance to the whole f'ing system that would allow an excellent physician and person such as myself to be psychology tortured for month after month. The case would also be dismissed if I was dead as well since you cant sue a dead person. Its not fair that the system allows someone who actually cares and tries very hard and is good at what he does to be tortured in such a fashion. One doctor from the hospital killed himself months back. Of course he was mourned by all (everybody becomes a wonderful person when they are dead)he was eulogized by many patients, little did they know that they were probably the ones that drove him to suicide. To have a potentially multimillion dollar lawsuit hanging over your head every day is a constant psychological torture and ordeal of severe mental anguish that was nearly too much for me to bear. It makes you call into question your personal skills as a physician, it makes you order hundreds of useless tests just to cover your sorry ass in case of another suit. The patient becomes the enemy and you become jaded and cynical and grow to hate the system and the profession that has made you comfortable. All these things I knew of happening to others and sure enough that is how I felt as well. I took it very personally and still do.Strangely enough I was sued by a lawyer from my own tiny town, since the family lives in said town. The Lawyer is also the township chief of the town I live in!! So I suspect that I will run into him some day. I also suspect that I know somebody who knows the family of the plaintiff given the degrees of separation that occurs within relatively small areas. The plaintiff lawyer tried to intimidate me in the deposition by acting like a jerk and threatening to keep me there all day unless I answered his stupid questions directly, I just smiled and gave him the answers as I pleased, knowing that my intellect far superseded this peon country lawyer and his welfare client. He backed down and the depo was over in 45 minutes. My lawyer said he wished all his clients were as calm and collected as I was, that I made an excellent witness. I think I would make an excellent lawyer actually, but my lawyer told me that my heart wouldn't be in it and he is probably right. The lawyers I have had experience with no little about medicine to be of any use. I could probably make lots of money by going over to the dark side and suing doctors.I developed an interesting defense mechanism during all this that perhaps helped me remain calm. I think it perhaps was from a symptom of stress that I experienced while going thru the ordeal. I would have somewhat intense separation experiences where I would feel as myself somewhat leaving my body and looking from the outside over the entire situation. When that happens the whole situation becomes somewhat more interesting and less traumatic I guess. I finally settled on the fact that if it didn't kill me that I really didn't care what happened since I would probably have enough money to at least retire to a trailer and buy a motorcycle and finally be free of this thankless profession. I also found that living well is the best revenge and I like to take my revenge with $300 dinners and Caribbean cruises. Although because of my hopelessness and depression I also stopped doing some of the things I like to do and also sold my nice sports car, my motorcycle, and all of my nice watches.Given all the bad phone calls, emails and mail I have gotten the last few years, I have developed a semi-phobia of reading email, answering my phone and going to the mail box. All of these things I now dread although 99% of messages received from these avenues are good news, fun stuff or junk mail. Mostly I just want to be left alone now. I did seek a help of a therapist locally and the help of my wife as well. Lucky for me the suit was dropped and I can breath again. Ironically my lawyer is 3 floors above my psychologist. So I would go to the lawyer for a meeting and then go down to see my therapist, a nice day well spent in downtown. The funny thing is that when I now tell people, they really don't seem to care that much, its like " too bad for you, now where do you want to go to lunch" Nobody really knows what it is like unless it is personally experienced to a certain degree. It is a trauma that I wont ever forget. Personally I am not sure how physicians can tolerate being in such a profession with such uncertainties. I would never recommend this profession to anybody knowing what I know now. I will now forever have to include a sheet of paper in any application and recredentialling application every year to the hospital detailing my ordeal to be judged by the peons in the suits who run the hospitals.For the most part I like the doctors and others I work with. I have an interesting job that I am very good at that few people can do. I get paid well and get time off and have much adulation from society in general. I however do hate the position I am in and wish it to end soon. Most physicians would not recommend their jobs and want to get out as soon as possible. I am one of them. The risk and stress are not worth it. I have made a goal of 10 more years then I am going to try and do something else. By that time I could probably work a job as a janitor and retire fine on what I have saved.What have I learned from all this? I guess I learned that freedom is what I value most, no matter how much money I have. It is freedom and self determination that I seek most of all. I also would like to help other physicians somehow going through this ordeal, but not sure how I can. I am perhaps more empathetic and more fatalistic about life in general. I am a very private person and dont tell people my business generally never. But I am telling some people about what has happend so they can perhaps know the truth of what happens in these cases, and perhaps experience a little joy for me in coming out relatively victorious from this horrible experience.Thank you."Now, my list or drawbacks:Increasing costs of malpractice insurance.Higher practice costs (i.e. overhead, tests).Lower insurance reimbursement rates.Less autonomy over how patients are cared for.Obamacare - the future is so uncertain, and politicians are increasingly involved with medicine.Malpractice law suits-- will incrase after 2014 along with defendive medicine, cuts, and work load complications on call, given the surge of newly insured patients and the already burdened physician workforce that isn't being expanded to help meet these incoming demands.Weekends, middle of night dealing with difficult patients and families.Many miserable colleagues.Ungrateful patients who expect care, which will only get worse with the incoming entitlement: Obamacare.More work per physician s/p 2014, with rounding on weekends, for less pay.Getting up earlier than anyone else to round on pts before clinic or surgery. It was worth it back in the day, but will it be worth it in the future?Medicine may be socialized in the future.Licensing issues--will our renewals be contingent on accepting the public health insurance option?Board re-certification-- when was the last time that an attorney had to take a bar re-certification exam, or an accountant had to re-certify? Why pysicians who aren't grandfathered in? (See item # 23 below.)Opportunity cost of delaying my earnings during another 4 years of post-graduate training, while my college roommate--who graduated 10 years ago-- has been bringing in half a mill in wall street for the last 4 years.Not being around for your kids as much as your non-physician fiends and relatives. Will I miss seeing them grow up?Making meager income slaving away 7 days a week while others enjoy life.Physician burn out.Physician stress & huge responsibility; considering items 1-18, is item #19 worth putting up with?One false move can cost you part of your family's financial future and your reputation.Less time for your hobbies and health. How many of you preach what you can't practice, and deal with the same medical problems that many of your patients have?Feeling like you've missed out on a huge part of your youth.Rules that apply to physicians, but not to other professionals.Being replaced by a "noctor" who has trained less, is less competent, but is being delegated the same responsibilities as many physicians because hospital administrators --and soon, the government-- will make a cost benefit decision on this front. Consider this is the case with midwives who are replacing OB/Gyn phsyicians, as well as CRNAs and nurse anesthetists whoa re replacing many anesthesiologists for less complicated cases that nonetheless could face catastrophic complications.Patients can be a pain some times, especially when they feel entitled to their care (kind of a redundancy here from another point made above, but well worth re-iterating).Fear of RAC auditors who get a commission to say we didn't see a patient that we actually spent more than the allotted time with because we failed to write "Reason for Visit - Scheduled F/U appt" at the top of the page on the chart.Pressure not to order tests/consults by hospitals and fear if we don't do these tests/consults we'll get sued.Terror at the thought of breaking the rules and being fined, losing license, going to jail, etc. because nobody even knows what the fricking rules ARE.Profound sense of learned helplessness. No matter what you do, you're screwed.Feeling isolated because non-physicians don't have a CLUE; they think all docs are wealthy, making money hand over fist, and thereby have no sympathy, but rather, they throw you in the same batch of wall street execs who they feel need to burn in hell.Feeling like being a doctor is like having a target that says "Scam me, I'm reeling in dough." And everybody from EMR vendors to office supply people want to charge as much as possible.Employees who embezzle.Having to hire a fleet of people to deal with the paperwork of running a practice.Knowing you sacrificed your entire life for this shait noted above and feeling like the biggest sucker on the planet for believing becoming a physician was a noble idea.Feeling absolutely and completely trapped. What alternatives do you have once you've completed all that long training, and realize that you want out, but don't want an entry level job in finance after you've started a family, are in your mid thirties, have worked harder than someone above you who graduated college 3 years ago, and are in more debt that you could ever wish upon your enemies?I could go on, but oh well... you get the point.

The Latvian Institute of Organic Synthesis developed Meldonium (Mildronate; 3-(2,2,2-trimethylhydrazinium_proprionate) in the late 1970s to promote growth in animals (1).A potent metabolic modulator, clinical studies showed Meldonium/Mildronate is anti-ischemic with effects ranging from cardioprotection, anti-atherosclerosis and anti-diabetes (2, 3). According to Sjakste and Kalvinsh (3), Meldonium/Mildronate is 'officially recommended for treatment of acute and chronic ischemic disorders of brain circulation in Latvia, Russia, Ukraine, Georgia, Kazakhstan, Azerbaijan, Byelorussia, Uzbekistan, Moldova and Kyrgyzstan'. A carnitine analog, it modulates its metabolism. Essential for the cell's energy metabolism, carnitine is the key molecule involved in mitochondrial fatty acid metabolism and as such has a potent effect on fatty acid beta-oxidation and glycolysis in the myocardium, the heart muscle (4; see figure below from 5).As far as sports use is concerned, effect of Meldonium/Mildronate on human physiology is similar to that of the prohibited Trimetazidine (6, 7), as both potently inhibit beta-oxidation of free fatty acids, and thus strongly influence cardiac metabolism.A recent review (8) states 'In dubious online shops, the performance-enhancing effects of Mildronate are advertised overtly' (9). In sports, taken at 0.25 to 1 gram twice a day for 2 to 3 weeks during training and for 10 to 14 days before competition, Meldonium/Mildronate showed increase in athletes' endurance metabolism, post-exercise rehabilitation and improvement in CNS function. These results were published in obscure journals not indexed by PubMed (10, 11). It may also improve mood, and learning and memory performance (3, 12).Though it was only in Jan 2016 that WADA (the World Anti-Doping Agency) placed it on its Prohibited List, Meldonium/Mildronate was already on its radar since January 2015 when it short-listed it for its Monitoring Program (13) to assess its prevalence and misuse in sports. In practical terms, the one year period was meant for developing methods capable of reliably measuring it to confirm its presence/absence in human urine. So at best it's disingenuous and at worst downright dissembling for Maria Sharapova and her team to pretend they were just unaware it was now a banned substance. From Jan 2015 they had more than enough time to find out. In addition, the Guardian reports (14),'makes the negligence at the crux of Sharapova’s story – that she received an email in December with a link detailing the updated banned substance list for 2016 but no one in her cadre of agents, coaches, managers and personal trainers apparently clicked it – so difficult to accept. It doesn’t help that other athletes have been caught after testing positive for the same substance over the past month, among them Ekaterina Bobrova and middle-distance runner Abeba Aregawi. Or that Russia’s anti-doping federation warned all Russian athletes of the new list back in September'According to the Guardian newspaper, 'The Latvian company that manufactures meldonium says the normal course of treatment for the drug is four to six weeks – not the 10 years that Maria Sharapova says she used the substance' (15).'Sharapova says she has been taking the drug for 10 years after she was regularly falling ill. She had a magnesium deficiency and family history of diabetes' (16).There seem to be no peer-reviewed scientific studies supporting Meldonium/Mildronate Rx for magnesium deficiency or as prophylactic for a non-diabetic with a family history of diabetes.Bibliography1.
