Some of you may recall I laid out how Biden would get the ‘national healthcare’ system in the US without calling it ‘single payer’ and without fundamentally changing the system.Today he took a big step back in that direction by opening enrollment and putting up to $1 billion for marketing, explaining and getting people on to the ACA (Obamacare).The numbers look like this:23 million signed up for the ACA and it stands at around 22 million today. It has not grown at all since Trump came in 4 years ago who did everything he could to kill the ACA.John McCain famously came to Washington while he was dying to ‘save’ the ACA with a vote against Trump a couple of years ago.Practically every week from his inauguration until he left office Trump said he had a healthcare plan but never put one together. I have written about this as the Republicans have no incentive to put together national healthcare as they cannot make money on it so they have no interest so people just die. It is that simple.Biden’s plan is:ACA - Open enrollment immediately increase by I estimate, 500,000 - 1 million in the next 3 months. He will reopen enrollment again as early as possible, probably in the fall and go for another 2 - 3 million as there are 26 million uninsured in the USA at the current time. He would like to have at least 25 million by the end of the year on the ACA. The goal over the next 4 years in my view will be to get the ACA to 35 - 40 million with ‘exchanges’ in every state backed by Medicaid. This is possible though some states will still block it.Expand Medicare age down from 65 - 60 - that will add 10 - 12 million on to Medicare and it will be mandatory if you do not have alternative coverage.Include everyone on SSI and SS in the system. The SSI number alone will add another 8 million although there is some overlap with current ACA participants.Include everyone who has a disability or pre-existing condition which the estimates are 80% of those 65 and older which is 46 million and growing 1–2 million per year.Again some numbers will overlap but you will quickly see that the numbers in the ‘public option’ (national healthcare) will be approaching 120 - 150 million in total by the end of his 1st term. Using only existing systems Biden will effectively increase those with health insurance subsidized by the government by 50 - 75%.This will get the ‘uninsured’ population down to a management 10 million or so which is a hardcore number that will need to be worked on over years perhaps with a different Medicaid solution or incentives for the young adults as penalties were rule out by SCOTUS.Currently the estimates are 175 million Americans are on some form health insurance that is not ACA or Medicaid or Medicare. Many may have temporarily lost their health insurance due to job losses with covid but this will come back.Combine the 2 numbers of private health cover mostly from employers and the ACA+Medicare and related pieces, you have around 300 - 320 million covered by some form of health care coverage in the USA.Is it perfect? No, but it will work and that is what counts and it will empty the emergency rooms when combined with other measures.There are many other pieces like big pharma reform, removing the prohibition on negotiating drug prices for Medicare, more local clinics, public/private partnerships and the rest.This is just a start but it will be a key goal of the Biden administration and his HHS pick Bacerra is one of the strongest supporters of the ACA in government so expect it to be a full on effort that will not abate from the Biden side.America is coming back on healthcare too. If he makes it to 320 million covered by health insurance including all of those with pre-existing conditions, then Joe Biden will have gone a long way to winning the healthcare wars in the USA.
