As a medical resident, a young man in his 30s had a “brainstem stroke” and ended up on a ventilator in the ICU. This part of the brain controls autonomic functions such as breathing, sleep/wake and the like. He was engaged to be married, and his fiancee was with him all the time. He as also completely estranged from his family, who were (bluntly) religious fanatics - he had not spoken to them in several years. As he had not signed a legal Power of Medical Attorney, the family took control of his care, praying over him and basically banned her from seeing him. He had no chance of ever waking up, but they refused to consider this - eventually, he was moved to a long care facility (where I am sure he spent his days never regaining consciousness until dying of pneumonia or some other kind of infection).
