My physiotherapist wants to use acupuncture needles; are there any risks related to this?

As background, I have a master’s degree in neurobiology and have practiced Chinese medicine for 34 years. Additionally I did my masters degree presentation (1982) on Acupuncture, Trigger Points and the Innervation of the Shu Points (points along the Bladder Meridian). I’ve written a brief answer to your question here and then included a letter I sent to several state legislatures to mitigate the issue of Physiotherapist’s attempt to co-opt acupuncture.Phyisotherapists and chiropractors have been adopting the use of acupuncture needles as a way of generating income under the auspices of “Dry Needling.” The theory of Chinese medicine has been elaborated 4000 years with at least a 2700 year textual basis that has evolved in every generation. In the US, a basic education in Chinese medicine qualifying one to sit for a national certification exam entails 1905 hours and 105 course credits.Chinese medicine cannot be reduced to a technique like “needle insertion.” It is the most highly evolved holistic theory of human function. It is an ecological medicine that understands human health in the context of body, mind, spirit, culture, and biosphere. It is not “needling” that makes Chinese medicine so valuable as a preventive and long term care alternative to drugs and surgeries. It is the world view upon which the medicine is based. You can just look at a paragraph of English and a paragraph of Chinese to see that two very different world views with comparative strengths and weaknesses created them.Physiotherapists are deluding themselves regarding the placement of acupuncture needles as “Dry Needling.” Some Physiotherapists denigrate the venerated theories of Chinese medicine and the scholarly literary tradition upon which they are based as “superstitious” in order to portray what they are doing as somehow “scientific.” This is ignorant nonsense. A trained practitioner of Chinese medicine can render an effective holistic treatment with 7–12 needles. Search the web to see the pictures of Physiotherapists inserting 25, 50, or more needles at a time merely to relieve “muscle spasms.” This is symptomatic treatment at best and it threatens to drive the source of these symptoms deeper. Then there’s the issue of safety. An acupuncturist has extensive training regarding safe needle insertion and a PT’s training is negligible. You would do yourself a favor by seeking out a seasoned, licensed, professional acupuncturist to experience the wonderful benefits of authentic Chinese medicine.Regards, Lonny JarrettLetter to State BoardsTo whom it may concern,I'm writing to express my concerns regarding allowing physical therapists to perform "dry needling" in your state. I have practiced Chinese medicine for 25 years, written two of the leading texts in the field, am a founder of the Acupuncture and Oriental Medicine Society of Massachusetts, and am on the continuing education faculty of many schools of Chinese medicine internationally (resume enclosed). Additionally, I hold a masters degree in Neurobiology and, in 1982, did my masters presentation on the neural innervation of trigger points and the relationship of these points to acupuncture. Trigger points were first needled during investigations of referred pain associated with angina and studies of rheumatism in the late 1940's and early 1950's. Scientists soon noticed that pain relief occurred even with the placement of "dry needles" without the injection of anesthetics.(1) What seemed to be important was the location of needle placement. In some of the studies it was commented that not only was needling effective for pain relief but that results warranted investigating if the technique of "dry needling" actually was evidencing improvement in the underlying conditions.Concurrent with Nixon's trip to China in 1972 James Reston, columnist for the New York Times, underwent an appendectomy and received acupuncture for his postoperative pain. His report in the Post aroused the interest of neuroscientists Patrick D. Wall and Ronald Melzack at McGill University in Montreal who were putting forward their "Gate Control Theory" of pain at that time. They noted a high correlation (71%) between the location of acupuncture points and trigger points in much of their research (2) and even a higher correlation (97%) has been noted in recent studies (3).While the Trigger point studies note only the use of points for pain relief, the high correlation in location and similarity of technique makes it clear that placing needles in, or near, acupuncture point locations and on meridian pathways precisely mimics the practice of acupuncture at a technical level. By licensing acupuncturists and holding them to a strict and rigorous standard of study and practice your state has recognized the serious medical implications, and necessary study warranted, for performing such a technique. Physical therapists receive no training in the deep medical implications of needling the points that they use so they possess little context regarding the broader implications of their actions on physiology. This can potentially lead to serious consequences for patients. Beyond that is the concern that allowing "dry needling" will open up the door for PT's to read a few books here and there or take a class and begin to mix the complex concepts of Chinese medicine with their own practice in a hodgepodge way that compromises the practices of Chinese medicine, physical therapy, and the well being and health of the patient. To sum, acupuncture by any other name is still the practice of a complex medicine that requires years of study to practice safely. It is my strong recommendation that the interests of the citizens in your state would be best protected by confining the use of acupuncture needles therapeutically to licensed acupuncturists and MD's.Sincerely, Lonny Jarrett

What can a Chinese speaker do to become fluent in English?

Understandably, Chinese speakers have some peculiar difficulties when trying to learn English. For one thing, English is a stress-timed language in which the stress of the sentence determines the rhythm of the language, while Chinese is a syllable-timed language. The alphabets, too, are different. This means that not only the languages look different on paper, they also sound very different, as English and Chinese have very few common sounds. This makes it that much more difficult for Chinese speakers to nail English pronunciation. So let’s go over some things that will help you become fluent in English.1. Practice self-listening and listening to native English speech. What I recommend to my Chinese speaking students is to listen extensively to native speech (podcasts, Youtube, the news, etc.) and compare the sounds that they produce themselves to native English sounds. Hearing the difference is an important step in overcoming the pronunciation barrier. While you listen, pay attention to the rhythm of speech as well and try to imitate it. Listen to short sections of native speech and repeat after the speakers. It’s a great way to get used to the unusual “music” of the language.2. Physical trainingPeople move their mouths in different ways when speaking Chinese as opposed to English. While speaking Chinese requires more of a closed mouth, English prefers a more open jaw position. This is why it’s important to actually train your mouth to produce the needed English sounds. Practice in front of the mirror and really make an effort with this one until it becomes second nature.3. ReadAn important part of achieving fluency is understanding the nuances of the language – slang words, idioms, etc. Reading is your gateway to that.Being fluent in a language means being aware of the culture as well, which is why you should read anything and everything to help you grasp those insights and connect the dots in your head.4. Practice with a native speakerFinally, when it comes to getting fluent in English, nothing beats practice with a native speaker. If you think you don’t have that opportunity, don’t worry – you can always do it online with English Ninjas. At English Ninjas, we match you with a native English speaking instructor, who will help you achieve that coveted fluency at your own convenience, as often as you need. As a Quora user, you will also get a special discount.

What are your views on the idea of scrapping away the JEE Advanced and letting students choose a campus over a branch for admission to IIT, as proposed by IIT council?

Short Answer: In all the probability they won't be able to scrap JEE advanced as there will always be a competition(maybe in another form or at other age) as long as there are less seats than the number of applicants. In my opinion, the IITs won't trust any other examination-conducting-body than itself. There are some voices in the IITs who think that quality of students admitting into IITs has decreased on an average after the subjective paper was discontinued.Removing JEE advanced to curb coaching-industry is not a intelligent thought as it will lead to chaos. Further, if bright students are not provided a competent platform then they may start searching for options abroad for an engineering degree.Very Long Answer : From the article, the proposal of scrapping IIT-JEE advanced is mainly based upon following arguments, reasons, observations and alternate proposed model(s):To stop students from starting the preparation very early(in 9th or 10th class). To reduce the competition or pressure over the students. To reduce suicide cases among the students who are preparing for JEE.Student will be allowed to choose the campus not the branch. The branch will be allotted after one year of study that is after judging aptitude and interest of the student. From JEE Mains first one lakh students will be selected without giving them any rank. Now these students will be treated as equal and will be allotted random colleges. To reduce influence of coaching.Stopping students from treating JEE advanced as ultimate goal. Many students are exhausted after extensive preparation or coaching.Coaching stress on rote learning and make it difficult for raw talent to qualify. It is unfair to subject 10 lakh children to many years of torture, in order to achieve a fine selection of about 5,000 students to the top IITs .In my opinion, the alternative method suggested or some of the reasons given are preposterous. You do whatever you like, still there will be competition and stress among the students. Until the time that the number of applicants is more than the number of seats available, there be competition and students will be in stress. Now I will try to give my views on the above mentioned points, point by point:The main reason for students to start early is that the IITs have restricted the number of attempts to 2 and earlier there was no limit. So students do not want to take the chances and start preparation bit early (but in my experience the number of such students is very very less rather negligible).Recently we celebrated historical moment when the team of 5-Indian students won 5-gold medals in physics olympiad(such things are not possible if students are not starting the preparation early). Also we celebrate the achievements of PV Sindhu with so much proud and you must know that she started playing at the age of 8 years. Stress is stress, be it physical or mental. On this line of thinking, should we stop people like PV Sindhu as well? You propose any alternate method which is fair and you cannot avoid the competition or stress.Let me give you a very recent example they cited the same reason when they removed board examination for class 10th now they have again introduced it, let me ask why?First of all to analyse suicide cases we should look at density rather than numbers for fair analysis. There are many suicide cases inside IITs also then using this logic we should close down all IITs as well. In my opinion suicide is more of a social issue and we should not attach it to JEE preparation. Just to give you a perspective gDP per capita in South Korea is approximately $36000 and that of India is $7800(PPP) also literacy rate of South Korea is 98 % and that of India is 74 %. But the suicide rate in South Korea is twice as that of India. Also the suicide rate in Puducherry is 80 Times that of Bihar(yes, you have read right) so should we rename Puducherry as Bihar. In my opinion to address this issue Social awareness and education of parents is required. In this case everybody will try to get old IITs and new IIT will oppose this, also the competition will not reduce as everybody will try to get IIT Bombay or IIT Delhi or IIT Kanpur and will work hard for it , so again stress. In this case the talent distribution between IITs will become very asymmetric and inside old reputed IITs there will be intense competition as after one year everybody will be fighting for so called good branches. Now this suggestion is very impractical in my opinion. This is one of the most foolish suggestion given consider a student who has won Olympiad medal at international level maybe studying in some Tumbuktu engineering college because of this lottery system and a student who has barely managed to get into first one lakh students might be studying in IIT Bombay. If system is not providing enough opportunity or good colleges to talented students then there may be brain drain even at college level. These bright students may go to west for engineering degreeThis is again laughable concern. If you want to reduce influence of coaching then you make a school system strong enough to prevent students from going to coaching. If system is unable to upgrade academic input provided at school then they cannot stop students from obtaining it outside. In ancient times Nalanda was a very sought after University and it had entrance test, the success ratio in which was around 20 % at that time. Adjoining the university there were many coaching centres(source is given in the last). So this coaching business is very ancient.Once the student has been admitted into an IIT, it is the responsibility of that IIT to keep them motivated and interested in the subject. It also depends the academic inputs or opportunities provided at IITs.On the contrary I can say " Practice makes a man perfect" or “The more you sweat in training(coaching), the less you bleed in combat(IITs)”By this standard the players who practice a lot before a tournament will be exhausted and won't be able to perform well in the tournament itself. Quite funny. Then the onus is on IIT to prepare such a paper which will bring out the raw talent or which will negate the effect of coaching. A paper can be designed in such way but it requires some effort from IIT's side.Without coaching ,a student who is studying in a village cannot fight with a student studying in a posh school in some metro cities.Again a preposterous argument. It happens everywhere. Recently approximately 2.8 crore people applied for 90000 jobs in railways so approximately for every seat there were 300 candidates. In IITs there are approximately 10000 seats for 1200000 applicants so for one seat there were 120 students. Buy this logic we should scrap railway recruitment. During selection in Army they take a running test and use it as a screening .Now there are many cases where the applicants faint and in some cases even die during this running test. Also I have seen that many youths in villages prepare for this running test for years(as many a times there are no vacancies) .Now out of these lacs of the people only few are selected. Going by the same logic we should also stop taking this test.In army a person will be useful only when he is physically fit similarly for a person to perform well in academic he should be mentally fit and should be able to take mental exertion. All in all if intentions are good then there might be some efforts to reduce stress among students. For which, in my opinion, social campaign is required rather than these outlandish ideas.Whatever they are planning to do, they should think very deeply otherwise they may have to roll it back like in the past (Introducing removing screening test,introducing/removing consideration of 12-th percentage while forming rank list,removing/introducing board examination for X-class students, etc. etc.)Hope I made some sense. Thanks for reading.Ek Bharat, Shreshtha Bharat.Sources :Nalanda according to Chinese students: A Talk by Sahana Singh (Coaching at the times of Nalanda University)Iit Suicide: Latest News, Photos, Videos on Iit Suicide - NDTV.COMList of countries by literacy rate - WikipediaList of states and union territories of India by suicide rate - WikipediaP.
