1. Education reform. College is expensive because a quality degree is in high demand. That, in turn, is because the value of a high school diploma from the U.S. public education system is perceived to be very low. Colleges have found that, even though school districts offer high school courses in chemistry, physics, algebra and even calculus, the college often has to "re-teach" these courses to incoming classes, because critical concepts were skipped at the high school level in an effort to make sure the lowest common denominator can follow along. Employers know this too. In the worst cases of the "no child left behind" mentality, kids with reading comprehension at a 5th-grade level are given high school diplomas.Therefore, I think the easiest way to reduce student loan debt, which is a major contributor to the current total debt load of Americans, is to reduce demand for college degrees among both employers and prospective hires by making a high school diploma mean something in the working world, the way it does in most other countries. Yes, there are universities in other countries, and you still need an MD to be a doctor, or a DJP to be a lawyer or judge, practically anywhere in the world. Perhaps more so, as it was lowering of credit restrictions that led to the housing bubble despite a spike in interest rates. When the bubble burst, and banks were only giving credit to those with perfect records, demand plunged even as interest rates followed. I think that making sure banks continue to demand the highest levels of creditworthiness from potential borrowers will do more to keep housing demand in line than any change in cost of capital.3. Medical billing reform. After mortgage default, medical debt is the leading cause of bankruptcy in the past 20 years. Obamacare made sure that everyone has health insurance, but did very little, outside some Medicare/Medicaid payment restructuring, to address the costs of healthcare; so, the only real change is that now the 85% of Americans who already had it are paying extra on their premiums to include the other 15% who couldn't have gotten it because it would have cost too much. Insurance also doesn't pay everything, especially the lower-level subsidized plans, so families at lower income levels can still be completely wiped out by one bout of severe pneumonia, not to mention a broken bone or the C-word.What's needed now is comprehensive billing reform; we should be compensating medical practitioners based on how effective they are at treating patients, not how much they poke, prod, puncture and X-ray them.Perhaps the first order of business is "no balance billing". This is the biggest scam in medicine; a provider who is not in a particular insurance network can still receive payments for services from the insurer, however they are not required to abide by the "allowable amount" that the insurer negotiates with network providers and can bill the patient however much more they want. Patients billed for out-of-network care often get the biggest bill of any patient, sometimes even more than uninsured patients (who get a "cash discount" from the provider's base rate to get them in the door), allowing the provider and/or the facility to beef up their profit margins as insurers increasingly squeeze them on allowable amounts for in-network and indigent patients.Here's the kicker; often a patient has no choice but to accept out-of-network care (a person who's been shot and lying on the street bleeding can't really afford to refuse the first ambulance that shows up because it's OON), or worse, doesn't know a particular provider out of a "team" of practitioners at a network facility is OON (the hospital, surgeon and nurses may all be network, but the anesthesiologist is not).Even insurers don't like these situations, and so they often force hospitals to sign "umbrella agreements"; any practitioner or service who delivers to or practices in that facility must agree to bill as if all care provided in the facility was in-network. The ACA also requires insurers to pay the higher in-network percentages to providers for "emergent care", whether they're in-network or not (but if they're not, the provider can still balance-bill).So, make that the norm for all insured patients; if the facility is in-network, or the patient doesn't have a choice because it's an emergency, all practitioners must accept in-network allowable amounts, and the insurer must pay in-network percentages. Patients can still face OON charges and balance-billing if they see an out-of-network provider for non-emergent care, and of course those without insurance foot the full bill on top of the non-coverage penalty, so patients still have a financial incentive to make good choices about their healthcare, in situations where they have such a choice.Beyond that, a move toward results-based billing is commonly advocated. A building contractor, for instance, gets their profit when the job's done; payments up to that point are usually materials and direct labor. The problem is that a patient is not a skyscraper; while the majority of problems have a relatively simple treatment that will result in success, any doctor will tell you it's not always that easy. This is especially true in cutting-edge surgical disciplines, like cardiothoracic and neuro (disciplines involving organs that just a few decades ago were widely labeled "noli me tangere"), as well as in psychological and psychiatric disciplines, and in oncology. These disciplines also happen to be among the most expensive areas of medicine as measured in dollars spent per successful outcome (which subsidizes all the failures for which a success-oriented billing scheme would not pay), with an honorable mention going to trauma care. Cost control in these areas is probably going to require further technological advancement, allowing these procedures and the technologies used to perform them to mature and increase their success rate.
