Yes.I’m a geriatric primary care nurse in the USA.I’m most ashamed of our health care system, but I often feel shame that I am a part of it….such a crooked, convoluted, profiteering system whose primary goal is not the health of its customers, but instead is the movement of as much money as possible.My country is rich. We have the resources to care for all at a higher standard of care than almost every other country, but we don’t. Even Medicare, the “insurance “ for the elderly and disabled leaves its patients with debt. Copays. Deductibles. Noncovered services. And one of the greatest outrages of so many, tier formularies for drugs. This is basically a system where drugs abc you can get as soon as it’s prescribed for a usually affordable price. But, the rest of the alphabet, nope. Your doctors office, read-me, has to fill out forms and dig through your chart to find proof you need this medication and not some older, cheaper drug. Often it’s not even really a cheaper drug to the general public, but your Medicare drug plan, (which is a separate insurance plan you have to choose and pay extra for), has some deal they made so the basically have access to this specific drug for low cost. Quite frequently the drug plan requests and has a right to copies of your otherwise protected, private health information. If your wife isn’t on this form we have you had to fill out correctly, I can’t even tell her if you are a patient here without opening myself up to huge fines and possible loss of my license, but your profiteering, soulless insurance company can read your entire chart of they want to. In a nutshell, the insurance is dictating which medications can be used. That is dictating care. Outrageous!Drugs , procedures, referrals to specialists, imaging tests are all frequently DENIED. Your health care provider says you need xyz, but your insurance company says no, we aren’t paying for it. You can still get xyz, but you have to pay for it. It’s the rare patient who can afford such. It’s an even rarer patient that, even if they could afford it, will agree to pay for it.Which is another area of shame I feel, more as a citizen and member of this culture than a nurse. Everyone wants “what’s coming to me “. The entitlement of my culture is all pervasive and overwhelming. “I want mine”. “I’m entitled to xyz”. But if their obstructive, profiteering insurance company refuses to pay for it, it’s pretty common that they get mad at me. Many people, especially seniors, have a choice which supplemental plans they buy. Pick the cheapest. Get the cheapest. Note as well that the process of picking a plan that covers all your current and future medical needs is nigh impossible. It’s truly a task well beyond people with little education or whom are suffering from an illness. I’m not sure I could do it.“I want mine”. Patients demand medication samples because their copay is too high, or often just more than they are willing to pay. Yet they smoke, are obese, are non compliant with medications and treatments. There is an all pervasive lack of responsibility here. People feel “free”, ie entitled to live shitty lifestyles. Obesity. Sedentary lifestyle. Smoking. Recreational drug use. Yet few accept any responsibility for the consequences of such. I hear this every single day. I am told our medication didn’t work, or asked for a pill or surgery to help them lose weight. Everyone wants a quick fix, but it’s gotta be free, because “I pay my premiums.”And what does our government do? Little to nothing. Medicare patients pay the most for their medications, because the government won’t allow them to use copay coupons. Medicare’s fees are the highest in the industry. The entire industry fee schedule is based on Medicare. Yes, health care here is an industry. It’s not a right. The poor, the non citizens, if sick will be stabilized if they present to an emergency room , but that’s it. There’s no chronic care offered. The ER will treat, for example, a diabetic crisis, but not the diabetes. Note too that the bills for that ER visit will be high, the highest fees of all.I also hear all day long people complaining about “obamacare “. There’s a common misconception that somehow it was the idea of health care for all those raised rates. It wasn’t. It was greed. It was the maintenance of the high profits kept in place. I’m telling you for what we already spend, we could cover everyone. Everyone! There is horrible waste now. Well insured patients demand MRI’s for acute back pain, yet scorn physical therapy or referrals to a dietitian to address those extra pounds their body must deal with that certainly contribute if not outright causes that back pain. Most back pain goes away with minimal treatment. Almost all of it wouldn’t occur if the patient was active and of a healthy weight. We spend almost nothing on prevention. No public service commercials about how to eat healthy, but omg the drug commercials are everywhere. Expensive drugs, marketed directly to the consumer.It’s a disgrace. It’s a waste. It’s an abuse of power, and almost nobody cares. Our congressional representatives? They get insurance for life. Certainly not the expensive, crappy plan I have. If you are healthy yourself, who cares about all those fat, lazy, sick, poor people? Poor people? Well, they just didn’t work hard enough, or their lazy, on drugs. Freeloaders. Why should I have to pay for them? I hear it all day long. From fat, rich, white Americans….Then it’s their turn. Now they are sick, and they are outraged to not have access to the best of everything. They paid their insurance premiums! It must be the fault of all those lazy minorities, in the “illegals “. It’s the Mexicans bringing in the measles, the whopping cough! Don’t you watch tv? Never mind the anti vaccination movement…. And while we are talking about vaccines, I don’t want that flu shot. I never get sick. So what about the fact that if everyone was vaccinated we could save lives….not my life.To those from other countries, know this. I might be labeled a liberal. I might be a minority, but I’m not the only one. People here need only to open their eyes….. We can change.
