How should I write a PhD progress report on my respective topic?

Every PhD student has to submit a progress report to his supervisor to let him (the supervisor) know what the student has so far done . Usually the progress report has to be submitted at every six months. You have to write a brief report of what you have so far done including the literature review, methodology, results and discussion and conclusion.
