This may be one of the biggest social problems that is quietly evolving in an American background of physician secrecy, burnout and stress. Today, I work with young physicians, residents and medical students to teach them basic financial literacy, proper income protection and offer life-coaching for the physicians I have as clients. If they were to become disabled, I offer advocacy through that process, so the doctor does not have to be alone… and trust me, as a disabled physician in America, you feel very alone. I was a trauma and cardiac anesthesiologist until 2010. As a disabled doctor who has also attended law school and business school my life has been very interesting educationally and full of many of the greatest instants a medical career has to offer, as well as some of the career’s worst moments.Physician suicide is at an all-time high currently. More than one doctor per day is choosing suicide and physicians are the #1 professional to both attempt and succeed at suicide. Physician early retirement is a growing phenomenon due to stress, burnout and a horrid sense of hopelessness on the part of our physicians towards their careers.In 2018 the number of medical students choosing “any surgical specialty” has fallen 50% since just 2010. That will create a debacle for our society 10-15 years from now… the coming shortage of talented surgical technicians will make the 2001-2005 shortage of anesthesiologists look like it was a just a cloudy day for hospital staffers to deal with and overcome. Physician Assistants and Nurse Practitioners will not be able to fill the surgical needs of America in that timeframe. Something will give, or something will break.Physicians first feel the stress of near inhuman perfection in medical school where the pressure to perform and outperform colleagues begins with exams, then the first steps of Boards and finally with The Match each and every March. In 2018 over 1,000 medical students went unmatched into the fields of their choice and had to compete for unmatched transitional Intern years (think grunt for a hospital) and the number of individuals on their 3rd year consecutive unmatched year has reached a new record in the USA. Why? Residency program accreditation to increase slots takes longer than expanding the number of medical students in the overall system. There exists the widest gap between the number of graduating medical students and number of available training positions even in the United States.The problem is not just big, it is small and personal. Dr. Robert Chu wrote a letter to both national medical officials and government leaders explaining the flawed system of the Match that undercut his career before it even started. Dr. Robert Chu went unmatched to residency. He committed suicide. That’s a loss for tens of thousands of his would-have-been patients. That was before training even began, some doctor’s suicide notes reveal a fear of harming a patient so much from extreme sleep deprivation or other despondent work-environment issues. Instead, utterly hopeless, they choose to kill themselves.Why do they regret their careers?Because they were lied to, that’s why. The entrepreneurial spirit of the American Doctor has been subverted by the 21st Century efficient ‘business’ of medicine. Factory-work medical assembly kills doctors’ dreams first and then their bodies and finally, their souls. In a last act of self-punishment, they die by their own learned hand. Highly intelligent, compassionate and expertly educated individuals cannot properly care for suffering people in 7.5 minutes in order to stay on-track for the business inventory- flow numbers that day. With milestones competitive in the business-practice of medicine; “inefficiency” or “low productivity” labels sometimes push doctors to continuously ‘feel’ like a failure in their own practices. Government mandates create soul-crushing environments for brilliant talented doctors with increasing pressure from insurance companies beating the doctor’s reimbursement into submission along the way. All they really want to do is take care of people!Many suicide letters and notes form doctors point to working conditions with simply inhuman expectations. One big universal complaint is the paperwork/electronic record time to patient time ratio that has, by some measures, completely ruined the practice of medical care from the physician’s perspective. And ‘physician suicide’ as an issue is not just an American problem or a gender problem. Male doctors shoot themselves, female doctors’ overdose, New York docs jump and docs in India and China hang themselves by ceiling fans. Educated in how the body works for over a decade credentials doctors to have a suicidal proficiency seldom seen in any other group of individuals.I did not escape this phenomenon, myself. After my disability, it took six years for me to hit bottom. I had to live knowing any ‘mistakes’ I made in returning to gainful employment would or could devastate my family’s finances. That just ‘killed’ me.Point in fact, male anesthesiologists are the highest number of physician suicides in America… and I was a male anesthesiologist. There was a study of ‘suicide attempts’ looking specifically at male anesthesiologists and the time it took between them deciding to try to kill themselves and their initiation of the suicide attempt… the answer was just minutes. They are found in their call rooms, in the operating rooms or they jump from the hospital parking lots… they have been found dead from overdose in their hospital chapels. They are showing those left behind where they were injured, hurt and let down.The secrecy surrounding physician suicide often leads to more doctors to think about or choose suicide as a method out of the mess of modern medical practice. We, physicians, are taught in medical school and residency to bury our emotional responses in order to be better diagnosticians and more efficient practitioners. We mask our emotions even to ourselves and so you get outwardly happy and content doctors returning from vacations killing themselves their next clinical day.The divorce rate is higher among doctors than the normal population. Child custody and care battles for more than a decade can erupt into nasty financial bouts between two emotional and psychologically injured people both using a physician’s income to sustain old wounds that won’t heal. No one enters medicine or a medical marriage thinking ‘I’m going to have to fight this highly educated, highly intelligent person for years over the kids.’ Infidelity is a known issue among doctors, as is the ‘skipping out’ on family issues including the death of family members or parenting duties to disabled children. You can hide a lot of your own pain and misery from yourself and your family by working 80-100 hours per week. Perversely, that incredible ‘work ethic’ leads to personal relationship dysfunction, infidelity, divorce, self- loathing and even suicide.In 11 years of treating patients in operating rooms… only one time did I lose my composure and let my feelings show in front of a patient’s family. Only once in more than 15,000 anesthetics. Thankfully, I was with an older doctor, the chairman of cardiothoracic surgery when it occurred, and he was kind in that moment, reminding me to never lose that compassion. When we lose patients under our care, we hurt…. and we hide that hurt from the outside world, we hide it from our colleagues, our families and ourselves. We practice this self-emotional-secrecy so many times it becomes habitual. Even in my own one-time case of emotional showing, I had done everything correct that day… I do not know why I lost my self-emotional-control. Yet, I still, to this day, sometimes wonder if I could have done more… like forgiving my-physician-self for doing my best is simply an impossibility.As I attended law school, I have assisted in many medical-legal situations generating a few dozen opinion letters both for plaintiffs and for the defenses of physician’s actions. I did this to be balanced and remove myself from leaning one direction or another on medical-legal issues. Yet, humans are mistake-machines during our lives no matter how much precaution is taken… especially unconscious mistakes. One of the last medical-legal cases I was involved with led to the discovery that the number and type of procedures being performed had shifted dramatically right after the previous employment contract had been completed between hospital and their little ‘factory workers’ called doctors. These physicians had ‘shifted’ their procedure numbers to maintain their incomes… consciously or unconsciously, doesn’t really matter because the expected number of errors rose dramatically… and in the particular case I was involved with, the changes had led to the death of a woman in her 30’s.Anything and everything a doctor screws up ends up a public story. The higher more-perfect standard one is held to in society, i.e. U.S. Doctors, the better the story of their collapse and public shaming is for society’s appetite to be entertained, enthralled and sickened by the actions of others. Sex, insurance fraud, DUIs, Medicare scams, greed, stupidity and loneliness all lead to poor decision making and the inevitable highly visible public shaming… all on a backdrop of the habitual inability to confront one’s own emotional state. Now with the Internet… these ‘human’ mistakes and the accompanying shame are forever for the American doctor.One big ‘advance’ was supposed to be the 80-hour work week! Begun on July 1, 2002, all medical trainees were prevented from working more than 80-hours in a given week or more than 28 hours in a row. I graduated in the late 1990’s. I was present for the 2002 switch-over… what happened in reality since the work still had to be completed is that all the residents went home at 5pm or 7pm depending on assigned hours restrictions and the attending physicians (me) just stayed until whenever to complete the work in the operating rooms. I recall working 100 hours per week as a resident and then, in late 2002, I was working 110 hours, even some 120-hour work weeks as an attending physician! Do I need to go into what sleep deprivation does to the human psyche?No one made me do it. That’s right, I chose to work 110 hours in a week. I could have just thrown my hands up and complained… but that’s just the problem, you can’t do that as a doctor in America.One bright spot pertaining that the work week reduction was the increased scrutiny concerning the maltreatment and hazing of younger residents by older ones in the pecking order of training programs… however this was coupled with necessary increased secrecy about lying about work hours so as to not get the training program in trouble with their accreditation bodies with violations on resident and intern time cards. I guess the bright spot was shoved back into the darkened corners of medical education and training. Worse, some resident and intern violators are then ‘outed’ semi-publicly within their residency program with ‘inefficiency’ or ‘low-productivity’ labels and are given the worst call schedules.Let’s back up for one moment. Doctors chose this career, right? They get paid great, right? They should just suck it up? Right? Unfortunately, that is exactly what they do, blaming and shaming go hand in hand pushing doctors to early career burnout. In the land of physician disability insurance, which I now inhabit, the greatest percent increase in claims illnesses are for mental/nervous conditions among doctors. The hopelessness and desperateness expressed by some doctors sounds almost incredulous unless you have walked in their shoes, which I have.My wife, a thoracic anesthesiologist, is a caring, compassionate and intelligent doctor. Sometimes the mercilessness medical system’s impositions and requirements and its accompanying political infrastructures seem unsympathetic towards the reality of physically practicing medicine properly. She works for what I consider one of the leading large medical institution in America. I know personally this system puts tremendous money and time into physician wellness… but they often miss the mark when it comes to application of those resources for their stated goals. Physicians just need to grin and bear the abuse, right?Normal people would scream by this point, “If you just can’t stand it anymore… get help… see a therapist… right?” Wrong. The confidentiality afforded regular Americans for psychological services does not exist for physicians. Even leaving town, paying cash and using a fake name sometimes cannot hide the treatment from the overall medical system, or your own colleagues. Your hospital, your own insurance carriers and even the medical board finds out you’ve sought therapy and then you are doubly screwed for trying to hide that fact from them. Confiding in colleagues does not work out well either, we are notoriously bad with ourselves as patients and even more so with colleagues because we believe we all have to just, ‘put up with it.’ Things are downplayed, rationalized away and any substantive discussion of true psychiatric issues are disregarded as weakness.Why do they regret their careers?Imagine knowing you have mental health issues and having nowhere to go and no one to talk to without making things worse for yourself, your patients, your family and your colleagues and friends… and left untreated and ignored, mental health deteriorates, and doctors choose suicide to escape the pain and the internal suffering. Do you know how humiliating it is for intelligent, compassionate, forthright and duty-bound individual who has championed for their patients to not use drugs and alcohol for years to be forced into random drug screening because the business of medicine wants cover from liability? For those suffering from emotional and psychological issues, coping with it via substances occurs very late in the disease state. If you think doctors do not know to avoid ‘testing positive’ then you are hopelessly naïve.Now some career specialties have more psychiatric issue than others; Emergency Department physicians and Anesthesiologists have higher rates of PTSD than other physicians. Whether as a soldier on the battle field or a doctor in the trauma bay or operating room… seeing human bodies apart unnaturally is a psychic-trauma for the caregivers. The cleaner the environment, think about a modern trauma bay, the starker the contrast of human misery are the traumatic gruesome moments in one’s medical career.What does society do after a physician suicide? Its hidden from the public and from other community physicians or explained in the newspapers as accidental death. Do you know what it is like to know your colleague hung themselves at home and have the community and newspapers all claim, ‘accidental death of this happy doctor’? Imagine answering questions for months about the loss to the community and having to lie about it. Medical schools cover up the suicide of its students because it hurts admissions. The obituaries claim ‘some accident’ or ‘passed away suddenly while asleep’ or worse, the newspaper mentions absolutely nothing about the cause of death… they just write about the happiness and accomplishments of the doctor, not the person.Are there solutions to the off-course nature of the 21st Century physician-career? Yes. They are complex and will take decades to categorically bring to the training and work environments of American physicians.I can’t fix the system. I start with the very simple idea of taking care of yourself first, before you take care of others. I try to assist each of my clients in the ways they need help. What I’ve noticed among young physicians and residents is that they are not taking care of the very basics of financial planning nor are they laying those foundational protections for their incomes. This is something I did very well myself and learned even more about how well I planned my life advocating on my own after disability. This is clunky, but will (probably) work.The trick here is to produce formulas which pull across the data from a source sheet you want to appear in the target sheet.We assume the source data is organized as actual data, row after row of names, addresses, or whatever information. Thus column A might have a name in row 1, row 2, row 3, etc, all nice and neat.If we were producing just ONE target document (or just one at a time), there would be no problem. Click a cell in the target, type an = or + sign, then click the corresponding cell from the source. EZ PZ.But we are going to produce multiple copies of the target, each one conformed to the data in the data file. Let's call these "docs" (short for "documents").The problem comes in when we reproduce copies of the original document to truly have a functioning merge which handles all the documents to be produced at one time. The only way to do this is to copy the rows and of our first document down the page as many times as there are merged documents to be created.For the sake of simplicity, let us say that each document is comprised of ten rows, such that:rows 1-10 = doc 111-20 = doc 221-3o = doc 3etc.The problem comes in with all the docs after the first, because the formulas that pull data will be based on their position with reference to their previous occurrence.Let us say that cell G3 in doc 1 = cell A1 in the source. If we were to copy that reference to cell G4 in doc 1, then Excel would automatically refer to cell A2 in the source. Each time you drop a row in the target, you also drop a row in the source.However, the reference in doc 2 will be 10 rows below the previous reference. Therefore Excel will pull data not from A3, but rather from the cell 10 rows deeper, A11, to keep the row-to-row correspondence. Likewise, doc 3 will pull from row 21, doc 3 from row 31, etc. Your merge will therefore include only every 10th row in the data. Annoying.So we need a way to tell Excel explicitly (but automatically) exactly what place in the data source to use. Here's way to do it:Let us assume that every doc will include the word "date," in column C. Thus if there are 30 docs, then the word "date" will appear a total of 30 times, always in column C (it could be any column, but always the same). And so, if we visually counted from the top down every occurrence of the word "date" we would know that when we counted the 5th occurrence, we would be looking at doc 5. We need a way to count the number of occurrences of "date" (or whatever word) from the very top of the sheet to the point at which the count is being calculated. That number will correspond to the row in the data sheet from which we want to extract the data. Enter a counting formula on any row of the doc, but OUTSIDE the area that will eventually be printed. Let us say that doc 1 takes up columns A through L, so we put the following formula in M2.="X"&countif($C1:C10,"date")X: refers to the column of the data sheet in which the data we want exists. It MUST be surrounded by " marks. Change X to the appropriate column in the data source.&: tells Excel to join the previous value ("X") to the next one (the result of the countif function)countif($C1:C10,"date"): Excel counts the number of occurrences of "date" found in the range of cell $C1 and C10. The $ tells Excel to make C1 the beginning of the range. But C10 will update to a different row number when the formula is copied down to other docs. Thus, in doc 2, the range will be $C1:C20.The result of this formula will be:doc 1 - X1doc 2 - X2doc 3 - X3etc.(X, of course, has been replaced by the corresponding column in the source data).Your formulas inside the doc that fetch data may therefore use the INDIRECT function to do the trick. Let us say that B4 should pull a name from the data. In B4 we would therefore enter:=indirect(M2)indirect: tells Excel to use the calculated value in M2 as if it were explicitly entered from the keyboard (or by clicking on the cell to which it refers). If you have 30 paper documents to create, then you will have 30 docs, arranged vertically, down the sheet. Here's what to do:Highlight all the cells in Doc1 INCLUDING the ones with the countif formulas. If there are 10 rows in the doc, then you will highlight 10 rows and as ever many columns as it takes to include your countif formulas. Copy your selection. The selection area will have a dashed line around it.Paste the selection immediately below the dotted line. Press Esc to unhighlight everything, then select doc 1 and doc 2 as a single block, covering everything from the first row of doc 1 to the last row of doc 2. When you paste this, you'll now have 4 docs. Select those 4, and paste to make 8. Repeat process so 8 becomes 16, 16 become 32, etc.Alternative method: Press Ctrl+end to find the last cell in the worksheet. If the last cell is NOT the bottom right cell of the doc, then delete rows and columns and save the document to make it so. Two more stories published in the Toronto Star add fuel to the fire and illustrate why Canada’s diplomatic mission in Chandigarh, Punjab, India does not serve Canada’s interest as it is a complete waste of money.The first one takes a look at the unscrupulous, unethical, lecherous face of immigration consultancy in the Indian state.Sandeep Ohri is a dashing 42-year-old who revels in zipping through the traffic chaos in his gleaming Mercedes, passing billboards touting him as the leading immigration consultant in Punjab state.But Canadian officials see someone else: an extraordinarily brazen and successful scam artist in an industry rife with deceptive swindlers willing to provide applicants with a litany of sham documents — everything from fake airline tickets and doctored bank statements to forged letters from Canadian-based funeral homes.Of the nearly 500 visa applications formally rejected this year, 228 come from Ohri and his firm, OGIC Immigration Consultants.The six visa-section staff who work at Canada’s mission in Chandigarh, Punjab’s capital, review 40,000 visa applications a year from students, family members and prospective immigrants. While official statistics aren’t available, one senior Canadian diplomat estimated at least a quarter of those applications are refused because of fraud.“More would be if the processing was completed, but sometimes you know it’s fraud and just refuse the request and close the file,” the diplomat said.India is Canada’s top source for immigrants simply because of the volume of applications the country produces due to the size of its population. Of those who immigrate, approximately half of them come from the state of Punjab. Punjab is home to the majority of India’s Sikhs. Thus there are more Sikhs in Canada than Hindus. Hindus comprise the majority of India’s population at 80%; Sikhs are only 2% (there are more Christians in India than Sikhs). In Canada, Sikhs constitute roughly 50% of the Indo-Canadian community whereas Hindus are just over 30%.In Punjab, “the average per-capita annual income of $484 is still the highest in the country” which means in relation to rest of their compatriots, Punjabis are not necessarily hurting. They have money.With that said we come to the second article. This one reveals the financial extent to which Punjabis go to send a son over-seas to secure “a better life” for the family. This can be realized in the form of remittances or the eventual importation of the entire family into western countries through family reunification schemes.Now, though the story is presented as a tragic tale of a Punjabi family who only tried to make a better life for themselves, I cannot say I share that sentiment. I believe they got what they deserved. You see, in India western citizenship is a status symbol and the article makes this clear.In the small village of Kapure, it is not about keeping up with the Joneses. It’s about keeping up with the Gills and Dhaliwals.On a narrow, dusty street, one house after the other boasts visible signs of prosperity — a fresh coat of paint, air conditioners, brick driveways and new cars. The children, playing hide and seek, wear Reebok and Nike.The neighbours share not only affluence but also a common source of it — almost every family has a son, son-in-law or nephew living abroad and sending money home.Every family, that is, except the Bhangus.In many villages, almost every house has at least one person in North America, England or Europe.Those that don’t, like the Bhangus, are considered pariahs, says Krishan Chand, who’s been studying the effects of immigration on villages with the Centre for Research in Rural and Industrial Development. They are excluded from events and, in some cases, parents are not able to find husbands for their daughters.Thus starts the chase where these left-out families will do anything in the hope of sending a family member — usually a son — abroad.This leaves them vulnerable to the smooth talk of unscrupulous immigration consultants. But the prospective immigrant is not a babe in the woods free of blame. They oftentimes participate in the fraud.Typically, agents provide prospective university and college students, and others, with fake bank statements and other doctored documents to support their visa requests, which are usually rejected without proof of one’s ability to pay school fees and living expenses.Others provide their clients with fake passports and visas. Most are rejected.A visa officer at the Chandigarh mission’s consular section told the Star’s Rick Westhead last year that the staff is deluged with applications sent with phony documents. “Over the past year, 85 per cent of employment letters related to work visa applications have been forgeries,” he said.The Bhangus find it hard to believe they are living this nightmare.Prabhjot (Bhangu) says he resisted for some years but then started exploring various avenues, but found he couldn’t immigrate to Canada under the points system, or as a student or on a work permit.The family knew there was a better chance of finding a well-to-do husband for Hardeep (their daughter) who had turned 20, if Prabhjot was abroad.That’s when Prabhjot says a friend from the village who now lives in Toronto introduced him to an immigration agent. The agent promised to take Prabhjot to Toronto for $48,000.Things go downhill from there but at least he escapes with his life. Not so for Amandeep Kaur Dhillion. Remember her? The status seeking impulses of her Sikh family back in India sacrificed her happiness and sold her into a loveless marriage so that they can (could) use her to immigrate to Canada. She was stabbed to death on January 1, 2009, her father-in-law was charged with first degree murder.Punjab is a hot bed of fraud and so, in their infinite political wisdom, the Liberal Party of Canada opened a foreign mission right in the belly of the beast. Opened in 2004 to reward Sikh voting blocs for their loyal support, Canada has the only foreign mission in Punjab’s state capital. With an annual operating budget of around $25 million a year the mission serves no purpose other than to waste Canadian tax dollars by funneling scam artists, the pathetic, the mediocre, the greedy, the aged, all into Canada. But they eventually vote and isn’t that what really matters in the end?Jason Kenney, Canada’s minister responsible for immigration, was in India recently to specifically address the immigration abuses Canada is subjected to by Indian applicants, particularly those in Punjab. Well, good luck with that. Why should India care? Why would India want to curb the steady outflow of non-resident Indians into foreign countries who can then influence domestic politics and the economy to satisfy India’s interests?(So) Forget India. Canada needs to protect itself and the way to do that is to shut down the damn, useless mission in Chandigarh. For one thing it plays into the hands of the immigration consultants in the state by making the execution of their scams all the easier. In this regard, Canada is an accomplice to the crimes.Also, what quality of immigrant is the mission helping to import into the nation when many of the applications are clearly fraudulent? If these people are willing to go so far as to defraud the Canadian government and the people it represents, then they don’t deserve to come to Canada at all. And what kind of Canadians will they be, if you can call them that, if Canadian citizenship is nothing more to them than a status symbol on par with an expensive car and fancy golf clubs?Canada needs to shut down the mission as well as curtail immigration from India altogether. We simply accept far too many immigrants from that country to be of any worth. And of that country, a considerable sum come from one particular region. Indeed, this is characteristic of the immigration system as a whole. Almost 60% of immigrants come from Asia and of that 60%, most come from a few source countries chiefly India, China, and the Philippines. Now Canada is holding the “ tooshy” the rear end of this filth.International students in fake marriage schemes to CanadaThe newspaper ads in India are the visible tip of a booming underground industry in fake marriages involving would-be international students.The prize for the “spouse” whose family buys an instant marriage with a foreign student is back-door access to a full-time job in Canada and a fast-track to citizenship.The matrimonial ads normally promise that the foreign students’ sham marriage, plus all travel and study expenses, will be paid for by the Indian families who are determined to have their son or daughter emigrate.The type of Indian student the ads seek is usually a teenage girl, who must have passed an English-language test and therefore be in line to be accepted as an international student.Media outlets in India, such as the Hindustan Times, report there is a “booming matrimony market for ‘brides’ who can earn the ‘groom’” coveted status as a migrant to a Western country.Canada is among the most sought-after destinations for Indian foreign students, say migration specialists, because it is the most generous toward foreign students and their spouses. Australia has also been popular, but recently tightened its rules.Here is a typical recent ad from one Punjabi-language newspaper in India, Ajit:“Jatt Sikh, boy, 24 years old, 5 feet 10 inches, needs girl with IELTS band 7. Marriage real or fake. Boy’s side will pay all expenses.”The ad is listed by a high-caste “Jatt” Sikh male, or more likely his parents. It seeks a contractual marriage with a young woman who has scored well (“band 7”) on an international exam called “IELTS,” the International English Language Testing System. Almost three million IELTS exams are conducted each year.Here is another ad, from the newspaper Jagbani:“Barbar Sikh, 24, 5 feet 8 inches. Finished Grade 12. Looking for BSc or IELTS pass girl. Boy’s side will pay all expenses to go to Canada.”In this ad the family of a lower-caste “Barbar Sikh” is seeking to have their son marry an Indian female with a bachelors of science degree, or a passing mark on the IELTS test, so their son can be allowed into Canada as her spouse.As these kinds of ads illustrate, the parents of the male “spouse” typically offer to cover all expenses for the international student, who often end up attending one of the scores of private colleges in Canada with low to non-existent standards.B.C. is home to 130,000 international students, the vast majority of whom are in Metro Vancouver, which has the highest concentration of foreign students in Canada.
