This is what happens in most cases, since there is a big hospital system in the picture.Lucy is the 'untethered' PHR. Not affiliated with any healthcare delivery organization. Hence, owned, fed, and maintained by the patient themselves.Untethered PHRs are the unicorns of HIT. Google Health, Microsoft Healthvault were the tech giant attempts at creating the mythical creature. There are scores of niche PHRs like CareDox (which started as MotherKnows.com - catering to moms worried about immunization records).To the second part of your question: "Is it a useful too?". Yes it is. For patients to become ePatients (empowered, enabled), they need to own and manage their data independently, without risks of loosing it when they change provider institutions.In a loose comparison, this is how internet email works. Work email inboxes get left behind (like MyChart) when you switch jobs. Your primary care likely has this information. If you’ve done a lot of moving from provider to provider this information might be scattered…I once had to contact the pediatric office I used to go to when I was a child to confirm my immunizations. Even if you are not a patient, those records exist somewhere. Some facilities may require you to speak to their health information department, while others may allow you to come by and pick up what they have.

HIPAAThere aren't central systems that contain an individuals entire medical record, which means that each doctor has their own record. In order for other people to gain access to this medical record, the patient (or guardian) needs to authorize the release of that health information to the other person or organization.Frankly, I don't see any reason for my school nurse (or the school in general) to have access to my child's protected health information. They only need relevant information, such as medications my child needs to take during school and immunization records. There is nothing else in my child's medical history that they need to be able to access, so I would not authorize the school nurse (or the school in general) to have access to the child's full medical record. It's none of their business and it's not information they need to be able to do their job.
