Work Medical Certification Form: Fill & Download for Free

This is something that may differ depending on the country of origin of the vehicle because the laws will differ. For the United States, the FAA has published the rules regarding commercial spaceflight, and while they require medical certificates for the crew, they do not require medical certificates for passengers. The primary requirement from the FAA is that all commercial providers clearly notify passengers (spaceflight participants) that space travel is not safe.That shifts the decision to the commercial providers, or maybe more accurately, their insurance companies and lawyers. Space travel will put stresses on the body, so I would expect at the very least that there will be restrictions much like those posted on signs outside amusement park rides. The insurance companies will likely not want to allow people that are pregnant, have heart disease, etc.

One of my colleague took leave without intimation for a week and when he came back he had a patch over his right eye and said that he was operated and couldn't inform.Manager asked him to bring the medical certificate the next day and he brought an old medical certificate and overwrote the date on it.When manger asked him, he said that the doctor was confused and he overwrote it.My manager enquired that the certificate says that the left eye is operated but he was having a patch on the right eye for which my colleague responded " When the doctor inspects my eye, my right eye will be his left" and that is why the certificate says left eye...!!!He was fired that day... !!!

Year 1982. That was just before my joining CIL in Nagpur. There is this formality before joining the company. We had to produce the ‘medical certificate’ from a doctor. There were ‘panel doctors’ from whom medical certificate could be obtained.Panel doctors include doctors in each locality of the city from whom employees can take treatment without paying any fees.Periodically these doctors would submit their bills(for consultancy charges) to the office and office would pay the bills to the doctors.(This used to be the system when there were no affiliated clinics for the office).This is a medical benefit given to the employees.Coming back to the story, I had visited one of the panel doctors for my medical examination.Before medical examination these were his words:“Now a days women are taking up office jobs. That is the reason for decreasing efficiency in office work”Those words hurt me bad. Because I was in a vulnerable position, I could not give any answer to him.Any how he gave medical certificate and I joined the office.For first few months I was assigned the job of passing the bills of these panel doctors.If there is even one discrepancy in the bill, I would write about it and submit it to my senior. My boss was an honest man.Once a bill of one the panel doctors came to me for checking. This was the doctor’s bill to whom I went for medical check up.There were many discrepancies in the bill. Total amount claimed by the doctor was not matching with the medical forms submitted by the employees.The bill amount was almost 50% more.Writing all the discrepancies, I submitted it to my boss. The next day I was called to his chamber. There this doctor was also sitting.As I entered and took my seat, the senior said “ Doctor is complaining that there is unreasonable deduction in his bill. Please explain why you have deducted the amount”Showing each and every entry and comparing each with forms submitted by the employees, I explained every thing to both senior and the doctor.“Doctor, I told you before. She is one of the efficient accountants in this office. I think now your doubts are cleared” said my senior.The doctor and I came out of the chamber.“Doctor, I think now your opinion of the efficiency of women is changed” I said smiling.He said nothing. Walked away with great speed.(Note: Had the bill not belonged to that particular doctor, I would have done the same thing with the bill)
