As a doctor, what was the most unorthodox procedure you ever had to perform on a patient?

I’m a gynecologist and surgeon now, but did obstetrics for 14 years. A friend and patient was 5 months pregnant and at high risk for premature delivery after a previous bout with cervical cancer. Sure enough, at one of her frequent OB visits, she had signs of incompetent cervix and when I checked, she was 7cm dilated and amniotic sac membranes bulging. The baby was too early to survive if born, so I decided to attempt a “Hail Mary” cerclage and stitch the cervix closed with a completely novel method. Miraculously, it closed her cervix and held up for 8 more weeks, and the baby was born early but healthy and now grown.Another unorthodox case was a cervical pregnancy, where the early pregnancy is misplaced and growing in the cervix rather than the uterus. It never works out and eventually miscarries and causes hemorrhagic bleeding requiring hysterectomy. Well, this patient was young and had never had a child and I wanted to save her from hysterectomy, so I fashioned a balloon-like surgical device used for GI bleeding into a pressure “plug” to stop cervical pregnancy bleeding. I offered the treatment option to the patient’s attending physician during the surgery to try and avoid hysterectomy. After many pints of blood transfusion and all other attempts to tie off bleeders and save the uterus, he finally agreed to let me insert the special balloon. I blew it up, and the bleeding slowed and then stopped. The next day we removed the balloon and still no bleeding. The patient went home a few days later, and 3 years later I read a medical report about her being the first case of a successful pregnancy after a previous advanced cervical pregnancy. That was in the 1980s and now the technique has become standard.

An example of an easy case: A physician friend (a physiatrist) referred a client who had been given a negligently fitted prosthetic leg by another practitioner. The negligent practitioner’s insurance company immediately contacted me and, after a short period of negotiation, settled for a large amount. Easy peasy. These quick settlements make up for the protracted, expensive cases that sometimes do not work out or turn out to be less than we expect.
