Basically, in variable there is a mechanism called TAP (NLTC or OLTC) which allows for variable turn ratios to be selected in discrete steps.Many times fixed transformer used in the step up & step down, noise in AC voltage will cause certain amount of loss and will help in aging of the transformer early.
