Here is one example: A person comes into the emergency room with left-flank pain. During the work-up, blood is drawn for the usual-suspect labs, and it is discovered the person is, indeed, diabetic as was stated in the patient history. Glucose is waaaay too high, and so insulin is given while pain control treatment is initiated, and now, due to confirmation of diabetes, other tests and scans are ordered and completed as various tests reveal other issues. By discharge, the list of problems is:Small kidney stone; provided pain medication and a strainerdiabetes; immediate treatment provided, dietary advice provided, told to see PCPCHD; note entered into electronic file for PCP to order thorough exam by cardiologistEarly DKD (diabetic kidney disease); see above substituting nephrologist for cardiologistHere’s the deal, though: This patient is uninsured; the ER visit is covered under EMTALA, but, because there is no insurance, and because this person is working poor with barely enough income to stay afloat, there is no PCP (primary care physician). No PCP means no follow-up care, no exams by specialists and, most importantly, no treatment for diabetes.This patient will be dead within a year, two max. And this patient will die within a couple years because s/he cannot afford medical insurance nor treatment.As to why this is allowed, well, in the US there is an unspoken ideology based upon the ancient-old idea that those who have the most- riches and power, usually- deserve therefore the most. If that is true, then it is easily reasoned those who have the least deserve the least. Couple this with the US interpretation of “free market” economics, and what you get is a society that appears to support the idea poor people, people who cannot afford to play in the free market sandbox, do not deserve to live.Oh, no one says that- not out loud, anyway- but that is exactly what happens, and is allowed to happen by law and regulation.
