Early in 2016 as a final year medical student, I broke my wrist. I was glad to have a reprieve from my psych ward posting. It was an awful experience. My physical pain was nothing compared to the anguish of helplessly observing in a psych ward.(Before I continue, let me clarify that psych treatment is absolutely needed to help people during some of the worst times of their life, so rather than deter people from getting help I hope that my answer will inspire doctors to treat their patients with more compassion. There are some conditions that are dangerous, they require hospitalization at times, and strict lifelong adherence to medication. Do not take this answer as a reason to avoid treatment for yourself or a loved one! Do your research and find a good facility).This hospital was in the UAE, but was run according to international standards. That’s the scary part.Let’s call the patient Mary.Mary was diagnosed as bipolar, she had experienced some major stress and had voluntarily admitted herself to the ward to get well again.Every morning Mary sat on a chair in the middle of a room, surrounded by a panel of 6 doctors, a nurse and 4 medical students.Every day they would ask the same questions.Mary was so drowsy from the medications that her head dropped forward regularly. Her eyes rolled back, her eyelids closed and she forgot the question she had just been asked. She was too tired to wash her hair, and the greasy strands hung over her face. She tried to reply clearly but it came out as a mumble.“Doctor I want to go home. I think I would do better if I was at home with my family.” She said multiple times during every single ‘session’.“You’re not well enough to go home yet. We will tell you when you are well enough. We will decide.” The doctor in charge would reply. No one spoke up. I sat there helplessly as a lowly medical student.At the end of each day I had to copy down the notes into my student log book.‘Patient not responsive.’‘Patient not willing to communicate.’‘Patient has depressed effect (affect ) (mood). Drowsy.’‘Patient combative. Not cooperating with treatment.’‘Not currently planning for discharge.’Every day it broke my heart. I wanted to scream at the doctors. I wanted to set Mary free like a bird in captivity. I didn’t. I was a little afraid they might admit me too.

Literally any way I wanted?About 50% of class time would be online. As in, students log in at home and watch some videos I created of the lessons, then show they understood it with a few online lessons. Then, sometimes, we’d have an arranged time to all video chat together, where I’d answer questions if they had any, or do some live teaching.The other 50% of the time, we’d meet in a traditional classroom setting for small-group work, or just to have a traditional class from time to time for variety’s sake.Online distance learning has two huge benefits:Students won’t be distracted by their classmates. That’s the #1 impediment to learning in the traditional school setting: other students and the distractions they bring.The material could be used year after year, freeing up time for more one-on-one instruction from the teacher.I’ve done a similar set-up before for that exact reason. I needed to work with a student one-on-one during class time, so I recorded the lesson I was going to give that day, and had the other students watch it at their own pace on their iPads, while I was busy with the other student. It worked well.
