I have advanced cataracts in both eyes, will cataract surgery really give me 20/20 vision?

Cataract surgery will remove any darkness and cloudiness you have due to your lenses becoming opaque and dark. We are born with perfectly clear lenses before sun and smoking plus aging, change the transparency. Cloudy, dark lenses are called “cataracts”. It’s good to have your lenses removed soon after cataracts start forming as they won’t stop forming…but with the good equipment used today, your advanced cataracts shouldn’t be a problem. Phaco emulsification done by an experienced ophthalmologist is a safe procedure.Once the cloudy “cataract” affected lenses are removed the new lens implants will give you 20/20 vision depending on what type of lens implant you choose….and also depending on whether or not you have any other “eye issues” affecting your vision.If your corneas are clear and healthy and if your retina and macula are healthy your vision should be very good! If you have astigmatism (double vision) this can be fixed during the cataract extraction (usually for an extra cost). Maybe one person in 30 have astigmatism.Most people choose “distance lens implants” which allows you very clear and sharp vision from about 18 inches out to as far as you can see. Then, when you want to read or do close up work, you would wear prescription reading glasses that give you 20/20 close up vision to read, sew, do what you like to do up close.With distance lenses you can see everything around you very clearly including being able to drive. Everything in the store, everything while you’re walking, everything about 18 inches from your eyes and beyond. My brother said he could “see like an eagle” when he had his cataracts out with distance lens implants.Close up lens implants are much more limiting as when you look up and out….you would see only blurriness around you. You wouldn’t have to wear glasses to read…but you would need glasses for distance vision…again everything 18 inches and beyond. You wouldn’t see clearly to even walk around your home without glasses.The newer lenses (last ten to fifteen years or so) have ranges of vision starting with close up and moving out in various sized increments. These implants have either circular segments or horizontal lines. You would need to get used to looking through different tiny pieces of the implant to see various distances from close to far. I’ve heard that some people really like these and adjust fairly quickly (I don’t know how long it takes…different person to person). There have been people who didn’t care for the constant adjustment they had to do to see all ranges of vision.These multi vision lenses are changing all the time and your doctor will explain the pros and cons of each brand of these lenses. They have probably become easier to use…but once you have your implants put in, it’s not good for your eyes, to have them removed and replaced, …..and ophthalmologists won’t do this unless there is a problem with the implant..they won’t remove and replace just because you change your mind…..so be sure you get all your questions answered before you decide on the lens implant you think you would like.You may want to talk to people who have the multi range lens implants to get a better idea of how long it might take to get used to them or how people like these lenses in general. Usually your ophthalmologist can refer you to prior patients who would be happy to talk to you about this lens choice. You could probably ask this question on Quora and get lots of answers from those who have them.Lens implants of any type have only been available for about maybe 40–45 years. When people had cataracts removed back in the 70’s it was because they only saw darkness. When the cloudy dark lens was removed they saw light again, and movement…but did not see objects or people…just light. Then, intraocular lens implants were developed so people could see everything they used to see.Anyway, choose the lens type most appropriate for you. Usually the distance lenses are included in the price of the surgery and insurance or Medicare pays for them. Then, you are measured for reading glasses for your close vision. The last I knew, these newer multi range vision lenses were not paid for by your insurance and most started at about $1,000 each or more.It’s been a few years since I worked in eye surgery so I’m telling you what I knew before I stopped. I doubt there are too many changes but there are probably even more lens implants to choose from. Just ask questions and do your research and you will be informed as to what you think would be good for you.The surgery itself is easy and quick with a topical anesthetic that works great. If you are having both cataracts removed you don’t have to wait more than a few days to a week to have one done then the other. If you have cataracts they are not going to get better so I think it’s good to have both removed with implants done closely together so you have two good eyes right away. You will have to put up to 4 eyedrops in your eyes for up to 4 weeks after surgery so the closer together…. the less weeks of giving yourself eyedrops.I wouldn’t bother with “laser cataract removal”….its much more expensive with lots of equipment that only does one part of the surgery…the capsullotomy. Experienced ophthalmologists do this without the laser, by hand, and the time taken is less. It depends, there is something called BCVA. Best Corrected Visual Acuity. The goal is always 20/20, we want you to see 20/20 and for the most part, a large amount of the population can be corrected to 20/20. There are people, for one reason or another that cannot achieve 20/20 with or without correction.There are several possibilities why someone cannot achieve 20/20 vision too many to list and most are beyond my scope of practice and understanding.Your eye doctor will always give you a diagnosis. If you are diagnosed with an accommodativea error then you need to see a vision therapist. Unfortunately, depending on the eye doctor it is possible that they simply just didn't tell you about vision therapy.Vision therapy can be viewed negatively by some in the eye care field. Some doctors do not believe there is a justifiable benefit to vision therapy. Not all eye doctors feel this way but the sentiment exists and I have witnessed it several times.There are really exciting advancements in the field of vision therapy that has shown remarkable improvement in visual acuity and binocular vision, even in adults.So if your error is accommodative there are options.There inability to achieve 20/20 may also be a retinal issue.
