These treatments completely changed all that.Imatinib, the first TKI, was developed by a team led by Dr. Brian Druker at Oregon Health Science University and was dubbed by Time magazine as a "magic bullet" for certain types of cancer. It's now used for other diseases as well. Patients with CML can readily achieve deep remission without going through many of the side effects associated with older chemotherapy treatments. Even if they are resistant to imatinib, which happens rarely, there are newer generations of this class that can cover the gaps. My brother had advanced disease at the time of his diagnosis, so now he is taking dasatinib, a newer TKI. Unfortunately there are still CML patients that become resistant to these newer treatments as well, so there is still work to be done.The development of imatinib is a classic example of translational research. Research at the bench had shown that there was a fusion protein kinase formed due to the specific chromosomal translocation found in all patients diagnosed with CML, and that this protein caused excessive proliferation of hematopoietic stem cells in the bone marrow. A drug to inhibit this defective protein was screened for by Druker's team, and fortunately its effects were very specific. The high specificity/low toxicity of a hematologic cancer drug was a new occurrence in drug development, and the end result was a compound that could virtually halt disease progression, and many patients end up going into full molecular remission. CML went from a life-ending disease to a chronic medical condition overnight.I was recently talking with my brother about all of this. He's very grateful for the work of scientists and clinicians because he most likely wouldn't be here today if these people didn't dedicate their time and effort to help those who are suffering. He is now considering going back to school to become an RN so he can do the same.
