THE RISK IS EXTREME -THE CONSEQUENCES CAN BE VERY TRAGIC - AND - DEPENDING ON YOUR OWN PERSONAL MORAL VALUE(S) -QUITE DEVASTATING !!!!!!!!!!!!!I personally know of two instances where a Patient denied - and signed the appropriate documentation affirming the denial - ANY chance of being Pregnant - and was subsequently found to be well within their First Trimester ( it was MOST LIKELY that BOTH Ladies were between 5 - 8 weeks pregnant - the Ladies both had histories of irregular menstrual cycles - so no “alarm bells” had registered with them that they might be pregnant).The end result was - after precise calculations of the dose received to the fetus by a Radiation Physicist - a Therpeutic Abortion for BOTH WOMEN DUE TO THE EXTREME DOSE THE FETUS HAD RECEIVED DURING THE FIRST TRIMESTER {EDIT; WAS OFFERED ( WITH AN HONEST EXPLANATION OF THE “PRO’s AND CON’s) -END EDIT}: AND WAS DONE- SUBSEQUENTLY both Patients underwent lengthy Professional Therapy as a resultMy colleagues who performed the CT exams - although having done everything “by the book” also were “grief stricken” for months afterwardThese incidents made a tremendous impression on me - and cemented my resolve on a point of Professional Practice ever sensePEOPLE - WHEN WE “QUIZ” YOU ABOUT ANY POSSIBILITY OF PREGNANCY -WE ARE NOT STICKING OUR NOSES INTO YOUR PRIVATE LIVES -WE ARE ACTUALLY TRYING TO PROTECT YOU FROM A POTENTIALLY TRAGIC CONSEQUENCE!!!!!In my Professional Practice - IF I have ANY Suspicion that a Patient MIGHT be Pregnant - I INSIST on a Pregnancy Test being obtained BEFORE I will scan the Patient -AND - I am NEVER -“NOT SUSPICIOUS” -IF - the Patient is of Childbearing Age!!!!I insist on having ANY Female Patient between the age of 12 - 56 fill out a Pregnancy Declaration Form - EVEN IF they state they have undergone sterilisation - are “on the pill” - have “a coil in” - have “been through menopause” etc.etc.The ONLY EXCEPTION is IF a Patient presents who is in a LEGITIMATE LIFE-THREATENING SITUATION (such as a Multiple Systems Trauma situation) - AND - the Supervising Doctor had put in WRITING -AND SIGNS- that the need for immediate information from the Scan on the Patient outweighs ANY potential risk to the Patient being Pregnant!!!!On a side note - it OFTEN can be very awkward having to get a Pregnancy Declaration on a Patient who is a Minor - and not legally able to sign the form - so a Parent or Guardian MUST sign for them. NORMALLY once I have explained there are MANY documented instances where females in the Patients Age Group have been Pregnant - the Parent and/or Guardian understands - but NOT alwaysI HAVE HAD Girls younger than 15 PRIVATELY ask me - outside the presence of their Parent(s)/Guardian - “what can happen” IF they are Pregnant - and they have the scanThis triggers a VERY ELABORATE COVERT EXERCISE on my partI contact the Senior Physician on Duty in the Emergency Department (A&E in the UK) and I discretely bring the Patient over and a Urine Pegnancy Test is performed - and YES - I have seen a result come back which was Positive - which has then led to a VERY Interesting discussion with the Parent(s)/Guardian as to WHY I cannot perform the Exam at this timeI EMPHASIZE my answer above is NOT MELODRAMATIC in Nature!!!!!ALSO - the use of Capitals in this answer is NOT me screaming at youIt merely is my way of adding “emphasis” to Points I am trying to makeRegardsFrank

What was the first surgery you ever saw as a medical student?

The first surgery I attended was in ObsGynae Department. We had no classes scheduled, so one of my friend suggested we should go to Gynae OT!(Adding this picture because answer was collapsed. Graphic content ahead!)Patient was not scheduled for routine surgery, but was brought to OT because a senior consultant felt her vitals were not normal!She was in her early 20s and was a case of ectopic pregnancy. SR in-charge felt there was no need of immediate surgery, and she made sure the whole OT knows what she feels! XDSo, patient was given GA, and surgery was started. While me and my two friends were standing there like chickens trying to see what was happening! 😂Incision was given and the first gush of blood came. I was expecting something special but it just felt like I have been there before, many times.It was a Tubal Pregnancy of 6 weeks and ma'am performed a Open Salpingectomy (removed the fallopian tube with the baby embryo).Then she removed something like this and placed it on Kidney Dish and said ‘Nurse ji dikha do in bachon ko!’ 😂It was very beautiful, actually! I think it is kept as a specimen in our pathology dept. now.One of the interesting things I felt in OT was the Instrument Count. After surgery was finished Nurse will check all the instruments and say -'Congratulations Doctor, your count is correct!’ And the surgeon would humbly reply 'Thank you, Nurse ji!’I didn't know Instrument Count was this official.That's it. XDThank you.Edit 1: I knew about Instrument Count, though I wasn't aware that it is declared this officially in OT. I just feel it is a nice practice.Edit 2:Ectopic pregnancy is an abnormal variant of pregnancy, in which the embryo is implanted at a site other than where it was supposed to be. Somewhere like fallopian tube!So, such a case needs surgery for removal of the ectopic pregnancy because the fallopian tube will eventually burst and the mother will die. Also, the baby wil never grow to it's normal size there. Lady was stable and fine after surgery and got discharged in time.

Summer of 2015My mother woke me up at 4 am. Her voice was whispery and shaky, as if she could hardly speak. My 17 year old self rushed to her aid, my younger siblings still asleep. She told me she needed an ambulance. My father was on a business trip and after she called him, he advised her to go to the hospital. Her left arm was numb, her chest was aching and she could hardly breathe. With shaky hands, I called an ambulance for her. She was 47 years old, and a few weeks into her pregnancy. I waited by her side as the paramedics came.I waited and waited. I had to care for my siblings. I was worried sick. What if she never returned from the hospital? Dark, negative thoughts filled my teenage mind in such ways I wish no child should ever have to think about their parents.When I woke up the next morning, she had returned. She was still exhausted, but okay. She told me that due to her age, her pregnancy was developing badly, and dangerously. She had a heart attack because of that problematic pregnancy in the ambulance. My mother thought she was going to die in that ambulance.Her pregnancy was terminated immediately. That possible pregnancy put her life in serious danger. I am glad I live in a country where abortions are legal, and can be performed by a professional, authorized doctor. That was my mother, but there are many women in similar positions, who would possibly be rejected from undergoing an abortion, even if that endangers her own life.I wholeheartedly believe that every woman should have the ability to choose to have a child, or terminate her pregnancy legally and safely. Because if we don’t provide her a safe termination, she may be putting hers or the fetus’s life in danger. Because she might try to terminate it in various ways, which are illegal, unsafe and unauthorized.For the mother and the child, abortions should be every woman’s choice. I honestly have no intention of sounding like a radical feminist, but I refuse to be told by a couple of conservative old men that it’s better to endanger my health and life if I happen to get pregnant, have problems during my pregnancy, or for many other reasons I want to terminate a pregnancy.
