A completed tag/title application, Form MV-1 - This application must be completed in detail in the name of the custodian (adult or trust company) and the name of the minor child – typed, electronically completed and printed or legibly hand printed in blue or black ink. If the custodian’s authorized agent signs the application, the authorization document (power of attorney) must be submitted.2. Transfer Documents - The original valid title or manufacturer’s statement of origin (MSO) must be submitted.
