She was nine, and wore her dark brown hair in an off-center ponytail, the sides of which frayed innocently around her head into a frizzy, caramel halo. Her grandmother, worried and silent, was too arthritic to ascend the exam table, but too proud to just give up, and so stood alongside instead, in opaque support hose wrinkling at the ankle.He was stubbornly far past retirement, in a bright white coat so heavily starched that I suspected it kept its own office hours. When he strode down the hall, nurses took little hops backward to accommodate his air of stern efficiency. Medical students hugging clipboards, myself among them, ducklinged in his wake.Crossing the hall, he snapped a chart up off the rack, and swept in without knocking to see the next patient waiting. I scurried along behind. He frowned momentarily at a manila envelope atop her chart stuffed with work-up from the referring PCP. Without another glance, it was banished to the back of the chart.He then addressed the tiny elderly woman, but failed to make eye contact. “Mrs. V, I’m Dr E, the neurologist. What seems to be the problem?” It was a question, but in his expressionless tone, that was difficult to tell.An uncomfortable pause passed before Mrs. V registered his prompt. Then, slightly startled, she chirped back, “Mande? Lo siento. No Ingles.” Her granddaughter translated: “She says, ‘Pardon, I’m sorry, but I can’t speak English.’”Dr. E’s head, once buried in the chart, practically ricocheted off the ceiling. I don’t recall the exact script of the tirade which followed, but the general gist could have subbed seamlessly into many a right-wing radio talk show. She was in the United States now! How long had she been here, anyway? 30 years? Why didn’t she bother learning GD English??!Her granddaughter faced the doctor without flinching, and translated all this to the patient automatically as it erupted from the neurologist’s mouth. But the child’s eyes gradually narrowed into laser-beaming slits intent on his own. When she was done interpreting, her lips tightened and she drew a long, slow deep breath in. The 4th grader and the senior citizen silently squared off. He blinked first.Head like an ostrich back down into the chart, he resumed, somewhat subdued, “So, please ask her, why she is here today.”She didn’t bother translating to her grandmother this time.“Actually,” she hissed instead, and her ponytail swished curtly, “My grandmother DID speak English. And then she had a stroke. And now she doesn’t, and that’s why we came to see YOU!”
