As a UKCP accredited psychotherapist with a Masters Degree in Psychotherapy, considerable clinical experience of the effects of childhood trauma and having studied this area in depth, I can categorically confirm that DID is real. I know several fellow professionals, highly respected in their field, who have also encountered individuals presenting with this disorder (whether or not 'officially' diagnosed - the mixed experiences of clients within NHS mental health settings and fears around being labelled/stigmatised probably need addressing under a different topic). I agree with the view expressed by other contributors that losing time is an important marker of a DID diagnosis.Sometimes during a clinical session, I or the client (or both of us) have been able to sense that dissociation is likely imminent as anxiety or distress escalates, or symptoms like emotional numbness / feeling 'unreal' set in. At other times, the switch has appeared to be instantaneous. I have observed clients switch to clearly different voices, posture, body language, mood and narrative; these so called 'alters' or different states of consciousness may exhibit behaviours or use vocabulary from an earlier developmental age. During such dissociative episodes, I have heard accounts of unimaginable forms of abuse and witnessed the sheer terror/rage/pain (emotional and physical) this memory has invoked, as if it is being replayed in the present. In my experience the primary identity's demeanour on 'coming back' looks like an awakening from sleep. It is apparent that no awareness of what occurred during the altered state is present. To onlookers, it is analogous to a door being opened on past experiences that is subsequently closed; amnesia regarding traumatic past events is reinstated. I have witnessed clients then become very agitated or frightened when they look at the clock and realise that time has passed for which they cannot account.The obvious vulnerability of such individuals, particularly outside of therapy sessions - dependent on the environment they are in and the consequent reactions of others present when they dissociate and then lose time - is a yet another anxiety that must be continually managed. Alongside this, clients frequently exist in a state of hyper-vigilant anxiety; something apparently innocuous to other people, such as being touched unexpectedly, may trigger in the individual a fragmented memory of past trauma. If the degree of emotional overwhelm does not precipitate dissociation to an altered state on that occasion, it may nevertheless feel as if the threat of harm is real and invoke a fight/flight type response.It is not only therapists, family members etc that can attest to the nature of 'alters'. I concur with other opinions expressed regarding the correlation between creativity and the capacity for employing dissociation as a defence mechanism in the face of overwhelming childhood trauma. I have seen many age-appropriate (to the relevant alter) drawings, written words or other outputs produced by adult clients whilst dissociated. From a therapeutic perspective, these are valuable in building up a coherent picture of the whole individual and their unique history but may in themselves cause further anxiety and distress, in that they appear alien to the primary identity.I hope my account has given a different perspective on this disorder. I would be happy to answer questions, with the caveat that I must scrupulously honour the confidentiality of the clients I have been privileged to accompany on their journeys.
