Will COVID-19 ever become the number one cause of death in the U.S.?

Click on an image to unblur it.It depends what time period you’re talking about.COVID-19 almost certainly was the number one cause of death in the US for about a month (April 2020), then dropped a couple of weeks later, and now is on its way back up again.There are around 647,000 deaths from the usual number-one killer: heart disease, in a year in the US. Heart disease is fairly non-seasonal, so it’s not far away from the truth to simply say that this equates to a flat rate of around 1,800 deaths a day, or a bit over 12,000 in a week.Using the normal counts (this is Worldometer, but Johns Hopkins gives you similar) and shoving this on the graph as a flat average gives you:So, it looks like COVID-19 made it to #1 in April. This is pretty extraordinary, given that heart disease accounts for around 1 in 4 of all US deaths, and also that this COVID spike was very localised — most of the country didn’t get much at all in April, and this spike was almost all driven by a few states, mostly in the North East (NY, NJ, MA, CT).Now, COVID counts can be disputed. Are we really, really sure that COVID was the absolute cause of death, rather than just someone testing positive for it and then dying of something else (heart disease, say)?A more robust way to measure the mortality impact of pandemics is with excess deaths. That is, regardless of what people put on the death certificate, how many more deaths were there in 2020 versus previous years?This one I’ve put together from National Center for Health Statistics Mortality Surveillance System, with weekly data this time, again for the whole US. It just has to get as bad as it was in April, but do so over a longer time period: something like nine or ten months would do it. Even if you believe that herd immunity might kick in and suppress the infection rates a while, then remember again that the numbers in April were driven by only a few states — a more widespread outbreak could persist for several months at a higher level without approaching anywhere near herd immunity levels.Unfortunately, current policies in many US states appear almost designed to test this assertion. The top (Worldometer) graph suggests there’s a new peak coming, and this one is driven by more, larger states (Florida, Texas, California, Arizona). Given that the case trajectory seems to be tracking ahead of the deaths peak by 5–6 weeks, it looks quite likely that the US as a whole will see more weeks of >12,000 COVID deaths in August.Contrary to some very confident claims, these things don’t tend to go away on their own. Something will have to change to get this rate down, and prevent COVID from being the leading cause of death in the US over a much longer period than a single month in 2020.
