I had to wait a day to collect my thoughts to answer this question. *sigh* Where do I begin?Short answer: this doctor needs a scribe.Long answer: The business of medicine is changing the practice of medicine. The only way to keep up in primary care is to use an electronic health record. During the Obama administration, several mandates were set in place to allow for bonuses for quality care and other feats of strength which pretty much replaced the decreased reimbursement. I don’t blame President Obama singularly since this was the aim even before he became president. In general, if you could measure something, you can change it. This is where data collection becomes ever important. On the grand scale nobody really cares about it, especially patients. The main ones we can impress are the number crunchers and data mongerers. The other issue is the fear of being sued. Yes, we do practice defense of medicine much more these days.Your “10 minute visit“ is really a 15 minute slot that saves five minutes for documentation and transition to the next patient. And by the way, all of that work being said, we also have to be on time and give service with a smile so we get good patient satisfaction ratings. It’s like juggling chainsaws, and yes while riding a unicycle.There is no such thing as “computer generated questions”. A computer does not generate those questions, someone with medical background has formatted those template questions to follow certain algorithms we already have established long ago. The question sets that are part of a visit template say, for a cold or abnormal vaginal bleeding, are part of routine questions that exist in our medical training. On the flip side, they are quite intollerant of medical treatment for drinking, very strict, and religious. Being atheist, this is a hard one to swallow. But many people have had success and it's the oldest program out there, meetings are basically everywhere. Some people are cool and some seem to be really hard core and intollerant. They rely on this long 12 step program and reference it all the time so you may get lost trying to listen to people speaking. They also have an odd “1 person speaks at a time without feedback” which I find very… impersonal. I had an AA guy tell me Friday if I didn't stop drinking non alcoholic beer I would relapse and fail to quit drinking. Now he's the only guy to say that but it is concerning to me that someone would view a non alcoholic drink as a gateway to failure. My wife has wine in the house, people have smoked pot around me, and I just drink my non alcoholic beer. So whatever.Clausthaler Dry Hopped NA is my favorite I used to drink double IPAs all the time and this keeps me satisfied, buzz free. That's another option for you when you're out a lot of bars have some kind of NA beer. If going to a party you can bring some NA beer yourself probably no one will touch it but you :)Now worst case scenario you have absolutely no control over yourself, you refuse to take medicine or get any help from the above programs, then you may need a psychiatrist to inject long lasting Antabuse into your system. That will literally turn alcohol into poison for you. The longest acting I believe requires a once a month injection or under skin implant. After drinking you will become incredibly sick. Sometimes even just 1 drink and you turn bright red, get a headache, vomit, etc. So now drinking alcohol becomes negative reinforcement and any pleasure you once had is gone. Some Asians can't drink alcohol because of having genetically this same condition. Apparently you and I don't have that.You have free will and can do whatever works for you thankfully. Those are all the options I'm aware of. I like NA beers and support groups to remind me why I don't want to drink (these guys have some awful stories). And I like scientifically backed methods to get what I want out of life using my own goals and aspirations.
