New employees can opt for CGHS services if they are working in CGHS covered cities. They first have to ensure that the monthly contribution amount for CGHS services is deducted from their salary, then apply for new CGHS card (serving employee) alongwith latest pay slip showing deduction of contributory amount. You can download the new CGHS card form from CGHS: Central Government Health Scheme or can apply online from http://cghs.nic.in. Then take a printout of the form and attach your current address proof, salary slip, proof of date of birth of all family members and get it forwarded from your office. Now submit this form at cghs headquarters of your city. Headquarters address can be found from cghs website.

It is unfortunate when head of the family passes away. Reality bites after a few days of grief. First step is to inform the last office where he was serving by giving application, enclosing copy of death certificate ( please obtain a number of deaths certificates) and please trust them for signing several pages of forms which the office staff give you , so that there is no delay or incomplete documentation. The deceased employee’s office would forward all the papers to competent higher authorities for progressing for family pension. Please ensure copy of Aadhaar card and opening page of bank Passbook for crediting family Pension. It is better to request for CGHS card for the Spouse and other dependent family members and don't opt for fixed medical allowance in lieu. It takes three months for getting family pension if all the documents submitted are in order.

To apply for Aadhaar, you have to visit the Aadhaar enrolment centre of your choice along with valid documents of proof of identity, address proof and date of birth. Don't forget to carry your original documents along with photocopies of the same to be submitted along with a duly filled-in enrolment form.
