I do not have my name on my birth certificate, instead there is a single "-". As written on the website, declaration on plain paper with parent's signature is sufficient for the passport. No need for the affidavit. They may ask you to submit declaration from yourself, something like:"I declare my name on the birth certificate as XYZ" with your signature below.Apart from this there is no other problem.
