My younger brother was an off-the-grid baby by accident.He was born in our upstairs bathroom after midnight. Apply for a birth certificate at your municipal office. If they have the records of your birth they'll issue you the birth certificate. In your case since it wasn't registered they'll issue a "Record Non Availbility" certificate. You have to take this certificate and contact a lawyer. They'll ask you to bring your school passing certifiable, voter ID card etc to prove that your dob is as stated by you. A lawyer files an application in the court and after verification a court order will be issued to issue you a birth certificate.Hope this helps.Source: many friends who got their birth certificates with this procedure and I have also spoken with municipal office and lawyers.

I do not have my name on my birth certificate, instead there is a single "-". As written on the website, declaration on plain paper with parent's signature is sufficient for the passport. No need for the affidavit. They may ask you to submit declaration from yourself, something like:"I declare my name on the birth certificate as XYZ" with your signature below.Apart from this there is no other problem. Go ahead with declaration on plain paper.I submitted parent's declaration on plain paper. I am pasting that draft here:This is to declare that I, <Father's name> am the applicant's father and applicant's mother's name is <Mother's name> and the applicant, our son, born on <Your birth-date>, is named <Your Name>. His name does not appear on birth certificate with registration number <Birth-Certificate Registration number>. Kindly treat this declaration as proof of name.(FATHER's sign)(Mother's sign)I received my passport last week.
