What is the first aid in an accident?

First aid to disaster victims, whether it’s man-made or natural, is organized under the almost universal system of mass-casualty incident (MCI) response. This is usually led by the emergency medical service (EMS) sector of the operational branch of the incident command system (ICS).[1][1][1][1][2][2][2][2] EMS activities in the operations include: triage, treatment, transportation and patient tracking (triage tags and destination records).Disaster raises an urgent need for rapid assessment & response, and the surge capacity to swiftly aid the barely predictable influx of victims. The system is designed while expecting that the number of victims are beyond the capacity of local healthcare facilities.The medical operations work by setting a rescue chain that starts from the scene size up by EMS that includes ready transport to prepared hospitals. By prepared, it means that the hospital emergency preparedness plan is executed. Part of the plan includes cancellation of all planned elective surgery, and calling the hospital staffs (surgeons, nurses etc) that are usually not on duty-calls.[3][3][3][3] The EMS approach the scene with law enforcement and SAR/fire crew (if needed) to seize the scene in order to conduct search & rescue, first aid, setting advanced medical post (AMP).Above: Medical rescue chain of mass-casualty incidents.The above picture is an example of a mass casualty incident scene size up: CP (command post), AMP (advanced medical post). The triage team approaching the scene can shout something like this:“I’m here to help, everyone who can get up and walk and needs medical attention, get up and move to ______ (pick an area). If you are uninjured and can help, follow me!!”And then you assess the rest of the people on the field with the algorithm below:Similar algorithm below:Tagging starts with the nearest victim. Immediate treatment such as putting pressure dressing on active bleeding is done during assessment. Everyone on the field is rapidly assessed and tagged by the first triage team with the color black, red, yellow, or green.[5][5][5][5] You move on to next victim one by one after each assessment.Immediate (Red tag): The victim needs immediate medical care and will not survive if not treated quickly. (for example, a major hemorrhagic wound/internal bleeding) Top priority for treatment.Delayed (Yellow tag): The victim needs medical care within 2-4 hours. Injuries may become life-threatening if ignored, but can wait until Red tags are treated. (for example, open fracture of femur without major hemorrhage)Minimal (Green tag): Generally stable and ambulatory (“walking wounded”) but may need some medical care. (for example, 2 broken fingers, sprained wrist)Expectant (Black tag): The victim is either deceased or is not expected to live. Fracture is splinted, airway is maintained if needed, bleeding controlled, stretcher and neck collar for suspected cervical fracture all while being transported to the medical post. No, CPR is not done on the field until it’s necessary on the medical post.At the AMP victims are placed according to it’s color tag. They are treated according to the needs. If one needs an evacuation an ambulance or choppers are usually on stand by at the evacuation path on the furthest back. Were there any civilizations in the ancient-medieval times that had common sense in medicine and the world around us, and weren't so cruel to people like Europe was?

As the other answers have already pointed out, the question reveals appalling ignorance of Medieval medical practices.Contrary to conventional wisdom, the standard of medical treatment in Medieval Europe was remarkably sophisticated given the level of knowledge about how the human body functions. It included highly complex procedures from hernia and cataract operations to (limited) brain surgery. Particularly during the period of the crusades, Western, Greek and Arab medical practitioners had an opportunity to compare medical theory and practice, each tradition benefiting from contact with alternative practices.I do not pretend to be an expert in medieval medicine, but below is a short summary of highlights I gleaned from Piers D. Mitchell’s seminal work Medicine in the Crusades: Warfare, Wounds and the Medieval Surgeon. There are, of course, other sources on medieval medical practice. (For example, Toni Mount’s Medieval Medicine: Its Mysteries and Science.) However, I found Mitchell to be the most comprehensive and convincing. Mitchell cites 172 primary sources, and 431 secondary sources. Given his own first-hand findings and subsequent research his conclusions cannot simply be dismissed. Piers D. Mitchell is an osteoarchaeologist.AnestheticsPerhaps most important, given the questioner’s allegation of “cruelty” (that implies the desire to hurt a person), the use of anesthetics during operations or the treatment of wounds was common in medieval Europe. An anesthetic was given to the patient either in a drink (usually wine) or placed on a sponge that was then held to his/her nose. Mitchell notes that the various plants recommended for preparing anesthetics (e.g. henbane, hemlock, poppy, deadly nightshade, mandragora root and lettuce seed to name a few) have been demonstrated to have pain-killing and or sedative effects. He hypothesizes that “cocktails” combining several of the recommended ingredients could have been very potent ― and dangerous if the dose was miscalculated or the extracts improperly prepared. Patients in the crusader states were lucky to have ready access to one of the most effective narcotics known to man: opium. Mitchell writes that there is evidence of its use for medicinal (rather than recreational) purposes by the Franks in the crusader states.Treating Traumatic InjuriesIn the 12 and 13th centuries, the weapons employed produced first and foremost puncture wounds (from arrows, lances and swords), followed by cuts and amputations caused by swords and axes, fractures/crushed bones caused by maces and stones thrown from siege engines, and, last but not least burns from Greek fire, boiling pitch and water. The fundamental (or first aid) treatment for each of these kinds of wounds does not differ significantly from what is recommended today.Medieval medical practitioners and soldiers, for example, understood the essential fact that a man can bleed to death. When treating puncture wounds, stopping hemorrhaging was, then as now, the primary concern. The difference between arteries and veins was likewise understood, and the need to stop arterial bleeding as rapidly as possible recognized. The use of tourniquet and precise cauterizing were both known, and surgeons were expected to be able to close off arterial bleeding with their fingers long enough to apply a cautery. Not only is the procedure for this carefully described in medical texts of the period, there are numerous recorded instances of men surviving this treatment and recovering so completely that they could fight again without impediment.While amputations were likewise cauterized and cuts bound, or if necessary, sewn back together, arrows presented additional problems. Although it would have been rare for an arrow to hit an artery, the arrow itself often remained in the wound and the need to remove it was paramount. But many arrows were designed to do more damage if pulled backwards (out the way they went in) by the addition of barbs or the shape of the arrow head itself. Medieval surgeons therefore had the option of pushing it through the injured man and out the other side, or waiting for the wound to putrefy and the surrounding tissue to become soft enough to make it easier to remove. Horrible as this sounds, the fact that many knights are described fighting with multiple arrows stuck into their armor suggests that it may have been comparatively rare for an arrow to become so deeply embedded that it was life-threatening ― without killing outright as in the case of arrows to the throat, eyes, armpits etc.In the case of broken bones, the need to set bones to ensure they mended straight and functional was likewise recognized. Bones were held in place by splints, bandaging or plaster ― or a combination thereof.In the case of burns, the primary concern was to prevent blisters from forming and the wound from completely drying. Moist cooling of the wound was thus the recommended treatment, whether by means of placing the affected limb in a bowl of liquid, applying wet compresses soaked in herbs or the application of ointments.InfectionWhile the fatal danger of infection was widely recognized and feared, the cause was not understood. As a result, some medieval medical practices contributed to infection. Once infection occurred, however, medieval doctors attempted to cure it. The successful use of vinegar, which has strong antiseptic properties, is recorded in treating festering wounds and severe burns, for example. Medieval doctors also understood the need to drain festering wounds. Below is the most selfish act I have ever EXPERIENCED in my whole life.My sister met with an accident by a vehicle and she became unconscious then and there. She was not even able to move her body an inch so she couldn’t take herself to the corner of the road. Seeing her condition some passerby took her to the corner of the road. One of the passersby knew my sister and us, so they informed me as quick as they could.It was a morning time, I was sleeping and as soon as I got this bad news, I rushed to her without thinking anything. All I could see was, my sister was moaning and groaning and was not in a condition to speak a single word. The blood was flowing from her body. (I can’t explain more how bad that scene was)From nowhere, a police car shown up and they asked me that they can drop my sister to the nearest hospital. Police dropped my sister to nearest Govt. hospital then informed to concerned Police officers and then they disappeared. (I am thankful to them, they did their best).Now the twist comes-In emergency ward, no-one from doctor’s team approached us. I approached one doctor and requested her, “Please give my sister a first aid treatment as nobody is attending her and it’s been a quite long time”. She replied in utterly bullshit way “Can’t you see there are other patients too? Is she different than others? go and wait”.After few minutes later I approached her again and she was just showing off that she is busy, so I asked another doctor who was sitting just behind that Doctor (both Doctor were listening), So second Doc replied in a quirky way “other patients need more attention, just wait there” Till now they didn’t even gave her a pain killer.This time I really got frustrated and decided to take my sister to another but Private hospital as nobody was paying heed to my sister’s critical condition. It was my first time I went to any Govt. hospital.Those concerned police asked us where we are going and then they DENIED it and told us that “we are not allowed to take my sister in any other hospital as it is a police case.”[Is police case more important than someone’s life? ]We informed police, that neither emergency team is ready to attend my sister nor you are allowing us to go somewhere else then what should we do now? Can’t you see my sister’s critical condition?He told us, all we can do is to wait as commanded by Doctor and as commanded by him. (These policemen were too bad and each one of them had a very bad temper. It was even impossible to talk to them).After one hour later they gave a painkiller injection. That’s it. (Nurse was too rude and slow in her process; it was like she was doing a courtesy rather than doing her duty.)The person who hit my sister was lying on another bed in same emergency room & he accepted his mistake and so we didn’t say anything to him but his relatives started abusing me, my elder mother & younger brother and even though they were at the fault. One guy started quarreling with us too bad in a high tone that my sister and other patients got disturbed by him. He also misbehaved with my mother and threatened us loudly that he will throw my sister from the stretcher to the ground. I was already in frustration, I said to him “you need not to shout, keep your tone low and she(my mother) is elder than you” then he said something foul language and went away.The final twist:After 2 minutes later, it was revealed that he was the head of the police constable in a casual dress. I thought he was also one of the relatives of the person (who hit my sister from his vehicle) as all the time he was taking their favor and was also standing beside them. I still don’t regret what I said to him because he was too noisy and very unkind person. (How can someone say or even think that he will throw out the patient who is already fighting for their life?) He was so callous that’s all I can say.He ordered all his police constables and local security guards to take me to his police post which was just at a walking distance. I kept requesting them and I said “I need to be with my sister right-now and once she will get her first aid treatment and gets fine then I am ready to go where-ever you like.”My elder mother and younger brother and kept pleading them but all in vain, those 14–15 people dragged me mercilessly to their police post and locked the door.Once door was locked, they all assaulted me physically and started mocking me for anything I answered to their question. That police-head kept threatening me all the time that he will now destroy my career [& he actually did, please read on] and he will do it for sure and he will show me how powerful he is. He told me clearly that he will charge me for those bad crimes which I never did and which I will never forget in my life. Then he said “It will take me years and years to prove myself innocent in the court and at the end nothing will happen and then I will realize to whom I have messed up with”, While talking, other police constables kept assaulting me physically like they badly needed a promotion or hike and wanted to show his boss how good they are.I kept requesting him, “Sir let me go, you was in casual dress and I didn’t recognize you and also I didn’t say anything bad to you and also what I said to you is justifiable as you know nobody is treating my sister and my sister is UN-attended rightnow and she needs me at this prime time of her life.” They all mocked me and he said “Its a government hospital, people die here everyday, we don’t give a shit about your sister. Staffs here do as we told them to do so. Once you will be behind the bars your attitude will be collapsed. Just look how many cases I will make against you. And you should have recognized me as I was in casual dress”.I replied, sir anyone can be in casual dress so that doesn’t mean…..He interrupted me and said--- I will make several cases against you and you will rot in prison.Then another police jeep came outside and two other policemen came inside the room, one policeman again assaulted me physically like I murdered someone. That Police head handed me over to those new policemen. Those two new policemen, and one constable who was with them from the beginning and who was physically assaulting me from the beginning also got in the same police jeep. Then they took me to the nearest police station.That head of police constable was regularly calling those policemen that I should get at-least 6 months or more jail term. At the beginning they also snatched my mobile.Here I am at the police station for no reason, and there my sister, is unattended and fighting for her life and getting no treatment from this Government hospital and police says they can’t give a f**k about it rather they are more interested to make my life hell and they wanted to ruin my career for no reason and eagerly wanted me to put me behind the bars rather than helping us.Once we reached outside of the police station’s building, they asked me Rs, 2000 as a bribe, openly.At this moment I realized, neither Mr. Arivind Kejriwal’s government nor Mr. Narendra Modi’s government did anything to improve anything in Delhi police.There my injured sister needed my help and here these policemen were very interested to make a multiple fake cases against me, so I agreed to pay them 500 rs (Because my sister’s life is more important to me than becoming a shallow abiding citizen). Seeing 500rs they got really angry at me and said that “Now even God will not be able to save me”. I really got afraid at this moment. So I gave them rs 1500 that’s all I had in cash at that time. They seemed satisfied. They let me go.I always have a clean record, never committed any crime, work in corporate and so I will never get a job, visa of any country or any govt. access because I will be counted as a criminal. Even if 1% I’ll be able to prove that I am innocent in court and court let me go free and released me from the case, still I will be counted as a criminal in my whole life.After wasting few more hours there, they let my sister go. We then shifted our sister to Private hospital right away. Doctors were very co-operative in that private hospital and treated us good and we thanked them a lot. Private Doctor informed us that my sister got a fracture in her spine and that part of the spinal cord completely collapsed & damaged, which will never re-shape to its original condition but can be healed but for that she must have at-least 2-3 months bed rest, that is why she was in pain all the time.(At govt. hospital they discharged my sister by falsely saying that she will be fine and she needs to visit them after 3 days as they gave her four tablets and a pain killer injection (Just for four tablets and a painkiller, govt. doctors took 6-7 hours). And here in private hospital X-ray clearly shows her spinal cord got damaged and collapsed completely and all treatment, X-rays, counseling etc got done in just 30 minutes).Please note- This is all what I said to the police head- “you need not to shout, keep your tone low and she(my mother) is elder than you” And in return what he has done to me. He wanted to ruin my life, physically assaulted me, whole time threatened me and uttered foul language to my injured sister, younger brother and old mother & mentally harassed all of us.Don’t I have my right to ask govt. doctors why they are not treating my sister?Don’t I have the right to get treatment in any private hospital?Have I done wrong getting rid of those inhuman and monsters by paying them and then helping my sister to shift to any private hospital where she gets treatment?Don’t I have the right to argue with a person (who is in casual dress) who is shouting & arguing using vulgar language with my mother and semi-conscious & injured sister, in front of me?Don’t I have the right to speak to my lawyer or any single person if police arrest me and seized my mobile?Is police case more important than someone’s life?Do you think it is acceptable what I have done?Do you think it is justifiable and acceptable what he has done to us in return?Police motto says “ Shanti, Seva, Nyaya” and,India motto says "Truth Alone Triumphs"These mottos seems irrelevant to me now after having this bad experience with Govt hospital, Govt Doctors and Policemen.UPDATE:-Thank you so much.
