I have not experienced it myself, but as a clinician having worked with numerous patients with labral tears, I will provide a clinical and evidence-based answer to your question.Although hip labral tears can happen insidiously or may be idiopathic or can be a standalone diagnosis, they are associated with something called as femoro-acetabular impingement (FAI) most of the times. I believe that understanding the difference between morphology and pathology surrounding femoral acetabular impingement (FAI) is imperative before discussing the necessity of surgery vs physical therapy management. A clinical viewpoint my Michael Reiman describes an 18-fold increase in the FAI surgery occurrence between 1999 and 2009 with just limited evidence supporting the cause-effect relationship between hip pain and FAI. In addition, there are currently no high-quality randomized studies examining the effectiveness of surgery for FAI. The viewpoint further states that surgical decision making for arthroscopic management of FAI has relied largely on clinical and radiological indications that are, as previously mentioned, inconsistent and largely invalidated. The Warwick agreement states that without treatment, symptoms of FAI will worsen over time and once a person seeks treatment for FAI, they are offered three different treatment pathways including conservative care, physical therapy, and surgery. Improvement in symptoms was maintained at 2 years after PT, for 5 years after surgery with some reports of the effectiveness of open surgery at 10 years. However, one should note that the research had poor design or high risk of bias. A study by Mansell NS compared the effectiveness of arthroscopic surgery vs physical therapy in patients with FAI. The authors inferred that participants patients had no change in pain, disability, or global rate of change score between surgical and physical therapy groups at the end of a two year follow up.Although the literature surrounding FAI is rife, clinicians and patients should consider the lack of high-quality evidence describing the efficacy of one mode of treatment over another. As the literature in this block suggests the discrepancy surrounding the diagnosis and treatment of FAI, not all patients may even need treatment in the first place. While surgery may be proposed as a quick remedy to pain and the evidence supporting either of the treatment intervention lacks quality, it may serve us best to focus on conservative management and PT before going under the knife until further research defines subgroups benefitting from these interventions.In other words, there is a roughly 50–50 chance that one may get better with physical therapy and exercises. It all depends on the patient and the extent of the tear. Out of nearly 8–9 patients that I have seen the past year, I have had one patient requiring arthroscopic debridement.
