PDF Editor FAQ

As a therapist, have you ever had a patient start taking their clothes off out of nowhere? How did you appropriately respond?

Yes. It was a brand new client I’d never seen before, and he wanted me to see more of him than I was ready for!I knew from his intake form that he was Australian, and had come to Virginia for an IT job. He was tall and lanky and wore a baseball cap and Hawaiian shirt. As he sat down on my slightly scuffed cream leather sofa, he started to quickly unbutton his shirt, and was reaching for his belt. Seconds passed before I was able to come up with the following highly empathic, brilliantly therapeutic intervention, expressing profound and unconditional positive regard:What are you doing!!! You can’t take your clothes off in here!I admit it. That’s what I said. Later I thought of all the things I could have said, but Carl Rogers would have congratulated me for concreteness and genuineness, I like to think. Probably not, but I like to think that.Anyhow, the client said calmly, “Would it be OK if I just took my shirt off? I’d feel more comfortable if I took my shirt off.”Still in full empathy mode, I blurted, “No!”Subdued, he ventured “Shoes?”Normally I’d be OK with shoes, but I just shook my head no.I would like to tell you that I quickly figured out what the disrobing was about, and what it meant, but I still have no clue. Neither did my peer supervisor, though he did remind me of the nude psychotherapy movement, and guessed the guy might have been involved in that earlier and made assumptions. I thought that was a huge stretch. (Yes, it was a thing: Nude psychotherapy - Wikipedia.)There’s no doubt in my mind that another therapist, someone a lot more talented than I am, could have handled that situation creatively and brilliantly and made it a powerful therapeutic event.

Is the healthcare system in the US actually as bad as some people say?

It is. I just returned to the US after 2 years in Puerto Vallarta (probably the most expensive city in Mexico). A doctors visit (to doctors who graduated from respected US medical schools) 1/4 the price. If you’re sick a doctor will make a house call. For about $75 he’ll be there in 20 minutes. You visit his office for a follow up at no added cost.Outside antibiotics and a few select medications, most pharma is OTC. The pharmacists are knowledgeable. The drugs themselves are 1/2 to 1/20th the price. My wife had a seizure and smashed her head — badly. An ambulance was there in 3 minutes. It cost me $150 (all in—no insurance). In hospital she had a CT scan and 2 attending doctors were wrapping up — before I finished the one page intake form. Altogether it cost about $400 for the hospitalization and 3 followups.Concierge level care at fair prices without hassle. Coming back to the US was a shock in many ways. None more than health care.US health care is such a scam it’s hard to believe it’s legal.

Have you ever observed a HIPAA violation as a patient or patient’s family member? Did you report it? What happened as a result?

My Dad was going into the hospital to have a stent placed after a heart attack. I rolled his wheelchair into the reception area and let the desk know he was there. That prompted an intake form with all his info to get printed which then went in a stack in a basket on the desk. As each Registrar came out they pulled the next form and called that patient back to be admitted. In the waiting area was another woman who kept pacing around and seemed quite agitated. As soon as the receptionist stepped away from the desk for a moment this woman grabbed all the forms out of the basket and began looking through them. She didn’t just glance at each one, she scanned them top to bottom. On the form was all of the personal info for each patient including diagnosis, procedure they were here for and all their insurance info. Having worked in medical research for over 25 years I knew how inappropriate and illegal her actions were. I confronted her and told her she couldn’t be looking at other patients information but she argued with me. She claimed she was just trying to figure out how soon she would be called back. I went and found a supervisor and told them what was happening and they confronted her and again she argued and got very angry. Next call was to security and she was escorted out of the building, never to be seen by me again. The hospital took it very seriously since it was their fault there was a breach in protocol (sensitive data left in a common area with no supervision) We got the best treatment from everyone (please don’t sue us) and I just laughed. My Dad did great but was NOT the easiest patient.🤣🤣
