For 6 years there were nothing suspicious to that lead to more than 2 months auditing.When I joined the it team, my first task was in the middle of upgrading process to copy all the financial auditing approvals from the old portal to a newer one. It was very simple just someone who knows how to write copy scripts and execute them nothing more, I spent one day writing and testing the script and getting approval for them and next day I got an execution appointment to execute every thing on production servers- which was frozen for the last 3 months and the new one is being used since then, they placed me on a terminal machine with remote server access. I just copied the scripts, opened the main file and pressed the run button. I spent the rest of the day just reading log files to make sure copying war going well. Finished my first task successfully, the next 2 days I was at lectures and then the week end. Everything looks good.The first day of the next week, the IT manager sent me a polite email saying that all the auditing information is missing from the new server and only the documents were copied and a 2 weeks old screenshot of the auditing information. I opened both the new and the old server and found nothing in the auditing data in both, so I replied with that and didn't receive any thing for 2 hours, so I assumed every thing is ok.I then got a forwarded email from him containing one sentence "what did you do exactly? " and a conversation of about 15 email from people I don't know saying that they can't find any information from the last 6 years, the same screenshot was present proving that the data was there and the only thing that happened was my work.I felt that I should run as fast as possible, but the IT manager himself left his office and came to my cube, he tried to be as calm as possible and not to freak me out but I think he failed. At this moment the whole floor was silently looking at the poor student whose professional reputation is going to end before it's beginning. I was told to try as much as I can to know what happened, look at the logs and got an hour before the big meeting.I got a remote access again opened my browser history to recall every step, but couldn't find any thing. I did a quick Google search about the problem, I found a 7 year old blog post saying the same thing. There is a server scheduled job on the portal server that runs every day and deletes everything that's 3 months old for performance issues and that the IT team should take care and either disable it or take monthly backups. I didn't find any other information about this problem anywhere else.I replied to the IT manager saying that we could check this service. He told me that no one ever mentioned it and called all the portal team for a quick meeting. I was asked to say what I found. They checked the job and it was running daily and it kept deleting all the auditing information each month, but no one noticed because no auditing took place more than 2 months old so no one complained, and the screenshot was for the last month of data that for my bad luck, read taken one week before getting deleted the day I was copying.At this point I didn't know whether I should stay or go, the IT manager asked me to take the rest of the day off and not to worry about anything it's not my fault. 3 of the portal team employees left the company immediately a huge punishment was given to the rest. I don't know what happened to the auditing dataI was pulled back to my startup and a replacement we sent to avoid any hard feeling towards me from the others. So I survived the situation but it war the most scaring thing ever to me.

I’d say that the main characteristic of a good doctor is that they are willing to devote the time necessary to help the patient with their problems even though they would get paid the same amount (or more) for delivering mediocre care.Doctors in the US are paid for procedures defined as CPT codes (Current Procedural Terminology codes). Each have a specified list of actions that must be performed to qualify for submitting that charge. The most common ones are those for consultation and continuing (or follow-up) care. These have five levels of complexity based on the components documented in the “note”. Today, these notes are placed in an electronic medical record so they can be submitted and then audited by clerks working at Medicare, Medicaid or an Insurance carrier. Thanks to computers, these notes can be generated from templates, with some additional information cut-and-pasted into it. What the doctor actually does with the patient is largely irrelevant to what ends up in the note. Since we are now required to generate a “note” on each patient if we want to get paid, we have much less time to spend actually interacting with the patient.In the old days, we would listen to the patient’s history, examine the pertinent organ systems, order the required lab or imaging and eventually come to a diagnosis. We would spend time counseling the patient and family members so they would understand the recommendations for overcoming the malady from which they were suffering.Our choice today is either to spend the necessary time with the patient or to do what we are required to do in order to get paid. If indeed we spend the time to do both, we can only see about half the patients we used to.In reality, a half page dictated note can summarize the information that any other doctor would need in order to know what is going on with the patient.Being human, with ever increasing costs for office overhead, malpractice insurance, etc. (not to mention supporting a family and putting kids through college), it’s tempting to skimp on the time for patient interaction and spend it generating a note that has all the requirements for maximum reimbursement.(Electronic Medical Records do have some advantages, but taking time away from patient care is certainly not a positive.)I think that today, an “above average” doctor is one who will take the time necessary to care for the patient, despite being financially punished for doing so.
