Very often they become medicaid residents of the nursing home.My grandmother sold her place, and eventually moved in with my aunt and uncle. She stayed there and judiciously used her savings. After a number of years she got tired of the rules that were imposed on her by my aunt and uncle and also was tired of having to rely on everyone else for rides. (She had sold her car, instead of buying another one.)She found a facility that had levels of care that you could get into and then stay through the levels of care. She rented an Assisted Living apartment and stayed in that until her money ran out. They then moved her into the nursing home into a medicaid bed. She stayed there until my aunt and uncle moved.Medicaid residents are using an allotted medicaid bed that is assigned to them by the state. Nursing homes have to be certified by the state and be allotted a number of beds that can be used by medicaid residents. The resident will assign their SS or another retirement benefit to the Nursing Home, and be given an allowance by the nursing home (usually about $120 a month for incidentals) The difference between the income from the resident and the cost is then billed to medicaid (the state)That is why when someone applies to become a resident of the nursing home they will ask private pay or medicaid. Very often they will have plenty of private pay rooms available, but are limited on the number of medicaid rooms.

An ambulance company I worked for knew that a guy in a rural area within their territory would die without regular transport to dialysis. Because he had medicaid or private pay (I can’t recall) and lived far enough out to put us at risk late call fees (since we operated with fewer than recommended ambulances for our volume, a not infrequent occurence), our company figured they’d never make their money back and refused to transport him. No attempt was made to get an arrangement to get the fees waived. They knew what they were doing when they decided, no ‘we didn’t know it was life or death’. Within a week, the medic who brought this patient’s case to management was taking them to the hospital as a cardiac arrest because he could not afford his care. Our management made a cost-benefit analysis of someone’s life and decided they weren’t worth it.

AN OLD WOMAN on medicare had a severe sudden hypertensive crisis.She was a FULL CODE PATIENT.I reported it to the charge nurse so she could get STAT ( immediate) orders from the doc for a med to reduce her b.p. .When she refused to call the doc,I confronted her and she told me it was none of my business, and to go back to the floor and do my job.I went back and took the pts b.p. again. It was UP on both sides another 10 points.I went back to the charge nurse and she gave me the same line.This happened a third time!I then called the d.n.s. at home and her response was identical to that of the charge nurse.At this point, I called the Doctors answering service and told them my name, the facility, the patients name and that I demanded a call back from the dr in MINUTES to save her life.The doc called back immediately, and spoke to me. I explained the situation to him.He went into a Rage, ordered the stat meds, and took other measures to deal with them.He had a lot of influence in the community, and they lost a lot of trust and business.If you do the math, its simple. Medicare pts pay less than private pay. If that bed “ became open” they could find a private pay pt to fill the bed and bring in more $.I told the dns she was morally insane, that I'd complete my shift, and after that she could put the job where the sun don't shine, BECAUSE I couldn't afford to have my name associated with theirs.The patient made a full recovery.
