Emergency Medicine physicians are not “surgeons”, even though our California state license says “Physician and Surgeon”. We work closely with surgeons, whose job is to take people to the operating room, where ER docs do not go.Emergency Medicine is a discipline that takes care of all complaints at all times, day or night, which is why the symbol for ACEP (the American College of Emergency Physicians) is an 8 by 8 box of dark squares with a single “lit” square near the center.ER doctors do see AND diagnose both medical problems (heart attack, pneumonia, stroke, rash, psychiatric, infections, etc) and surgical problems (lacerations, broken bones, sprains, eye injuries, gunshot wounds, stab wounds, appendicitis, thoracic aortic dissections, etc.), and call in appropriate specialists to take over.In medical cases, we will start the workup and treatment and when needed call in the specialist, such as the cardiologist, neurologist, internal medicine specialist, pediatrician, pulmonologist, nephrologist, gastroenterologist, psychiatrist, etc.In trauma cases, we will start the workup and treatment and when needed call in the trauma surgeon, general surgeon, neurosurgeon, orthopedic surgeon, eye surgeon, vascular surgeon, thoracic surgeon, orthopedic surgeon, ENT surgeon, urologist, gynecologist, etc., as needed. The ER doctor remains the “jack of all trades” and calls in the appropriate specialist for any more involved emergency.There are no “surgeons in emergency medicine” per se.Now to answer the question, sudden death in the pediatric population is always the hardest. This might be a child accidentally run over by his father backing up the family car in the driveway, the SIDS infant who cannot be resuscitated, the teenager found at the bottom of the pool a few minutes too late, the 5 year old who thought mom’s iron pills were M&M’s and overdosed days before and died in the ER, the little girl whose mother thought she could best treat her fever with adult strength Tylenol all night long every 2 hours causing the child to present with liver failure and requiring a liver transplant, the 13 year old boy who ran out behind the school bus and was hit by an older lady who was not paying attention, the 7 year old girl tied to the loose saddle of her horse which spooked with her flipping under the horse who then jumped over a wire fence and nearly cut her head off, etc. I’ve seen all of these cases in the last 35 years as an ER doc and they are all very painful, mostly because they were children who did not get a chance to experience a full life.These experiences, however, are offset by the lives I’ve helped improve or even save. The best part is when somebody comes up to me (like last night) and says, “You were the one who saved me when I had my heart attack”.Hope this helps.
