Should we require a person to get a medical clearance from their doctor before they can purchase of posses a car?

The police brought him in for medical clearance before going to jail. We get a lot of “medical clearance” patients in the ER. It’s important to ensure there isn’t some life threatening medical condition before placing someone in psychiatric custody and jail. This is because once you start being evaluated for a primarily psychological problem they aren’t thinking much about other medical problems. In jail… well, let’s just say they may not give your physical complaints a lot of credence. Not everyone going to an in-patient psych facility or jail will come to see us first. Typically, it is someone acting strangely or complaining of something like chest pain. There is a lot of controversy around the practice to be honest but I generally don’t mind these cases because they typically don’t require a lot of resources.I went into the room with the chart that said “medical clearance for jail” and saw my patient lying on a ER bed with one arm handcuffed to the bed frame. There were two cops on the far side of the bed and I recognized them both. They were senior officers that had been in the ER a lot over the years. It was strange to see them both together because this city had single officer units. That meant there was something concerning about my patient. He was lying there absolutely calm and still. He didn’t appear injured in any way. He was mid thirties and pretty out of shape. He had a dirty, white t-shirt and dark sweat pants on. He had on flip flops. I noticed then his feet were covered with dried blood.I asked the patient why he was there and he just ignored me. So, I turned to the cops.“Well” one of them began, “ he attacked his mother with a samurai sword.”I was kind of shocked by this. I asked the cops if they had to taze him and they said “no, when we got there he crouched on the front porch and placed his sword down in front of him and we just cuffed him.” I asked them why they brought him in and they said it’s because he won’t talk.I examined the patient the best I could while handcuffed and he didn’t seem to have any injuries. I tried talking to him and he wouldn’t respond to me. He just kept looking straight ahead. I asked the cops about the Mom and they told me she was taken to the trauma center but she was pretty hurt. She was in her late 60’s and poor health.I then turned to the patient and I asked him “what made you attack your elderly mother with a sword?” He looked me straight in the eyes and said “she was not bushido.” I asked “what is bushido?” “The warrior way” he replied. I thought for a moment and asked him “so, is it bushido to attack an unarmed and weak opponent with a sword?”At this point the guy leaped cleanly out of the bed and in one smooth motion somehow had me pinned against the wall where he began to deliver a series of knee strikes to my hip and abdomen area. He grabbed me with his free hand. I grabbed his hand and fell purposely to the ground pulling his cuffed arm over the bed rail and dislocated his shoulder. The bed turned over. The cops were now on top of all of this but we were tangled on the floor and he was seriously trying to kill me. It only lasted for a few minutes but it felt like forever.Another doctor and a nurse came into the room and we had to paralyze the guy with medication to get him under control. Then we had a medical emergency on our hands. He was ultimately placed on a ventilator, restrained and placed in the ICU. He was released in a day or so and he went to prison. His mother lived. In addition to charges concerning her assault he was also charged with assault on me and the two officers.So, what did I learn here? Your life can be in jeopardy in a heartbeat. “Why” questions in medicine are many times not worth asking. Never underestimate someone’s capacity for violence. I also never placed myself in a treatment room where I could get trapped like that again.
