> Why do people complain about the American medical system?Because it's broken.Medicine in the US is great when it is at its best, and at worst horrifyingly bad. The primary reason for this is that the system has evolved into a mechanism for extraction of money for people outside the practice of medicine.I will provide just a few examples from personal experience.I have on many occasions showed up at the pharmacy to pick up prescriptions, only to be told that the prescriptions were not filled because the insurance company had changed the formulary. If the prescription in question is acne cream, I'm not too worried. But both of my children are Type 1 diabetic and withholding insulin is potentially fatal in a short period of time. Did the pharmacy believe we'd simply do without? Even if we had to pay, we need the insulin so fill it, please.Furthermore, you don't switch types of insulin unsupervised. Making a change like that is something the doctor and patient plan in advance and watch closely during the transition. It isn't supposed to happen without notice because the insurance plan administrator makes a policy decision. Even if that were acceptable, insulin delivered via infusion pump is even more difficult to transition and requires medical supervision in all cases.When I transitioned from company insurance, the procedures prohibit applying for COBRA (continuation of benefits after terminating employment) until after my last day at work. It takes a week to get the forms, and a week minimum to process them. For people who fill them out the day they arrive and then return them in the next day's mail, there is a minimum of 2 weeks during which it appears to the pharmacy that the person has no insurance.In my case we had some prescriptions to fill during that time and I told my wife to pay for them in cash and we'd get reimbursed. Federal law says that under COBRA the ex-employee is entitled to the same benefits and same level of service they enjoyed as an employee. These prescriptions had never cost more than $20 combined so I figured I'd get back all but $20 of whatever we paid. I was wrong.The self-pay cost of these prescriptions was more than $500. The insurance company applied the reimbursement on an 80/20 plan with deductibles, leaving me with the bulk of the $500 outstanding. I appealed on the basis that, according to Federal law, my out-of-pocket cost should not be different than it was as an employee. It took three levels of appeal and more than 30 hours of administrative and phone time to get the reimbursement.Think about what that means: for my insurer to comply with Federal Law is an exception requiring three levels of escalated review. Their standard operating procedure is in open defiance of that law and compliance is reserved only those with the resources to fight. Here in the US you get all the healthcare (and justice) that you can afford, and not a bit more.The same incident revealed a different US healthcare injustice. My insurance company had negotiated the price of the two medications so low that they never had to reimburse for them. We had a $10 co-pay. That means we pay $10 at most, but any drugs that cost less than the co-pay we pay the full amount.As an insured, our cost for these two prescriptions had consistently been slightly more than $10 for both. As self-payers, our cost was well over $500.If you remember, the idea of an insurance company is to spread risk evenly across a pool of insureds. Most people pay in more than they claim but this relieves them of potentially catastrophic risk.But in this case the insurance company has so many covered members that they are able to negotiate discounts so deep that the pharmacy shifted the burden to the uninsured who have no such collective bargaining power. Given the cost differential, the uninsured buying these specific medications are subsidizing the insurance company at a rate of 50-to-one.Yes, you read that right. Rather than keeping the cost within their own risk pool, the insurance company has found a way to shift cost completely outside their risk pool such that uninsured people now subsidize the company. Though we do not have debtor's prisons, we do have collection agencies who file in court to collect the massive medical debts of the uninsured, issue subpoenas, which become arrest warrants if unanswered. We then jail the people involved until they pay off their medical debt, at prison labor rates. Which is why I gave my blog post on this the title American slavery - it's a thing.If you were in my shoes, wouldn't you complain about the American medical system? As a foreign doctor you are probably better off to practice in any country with decent medical schools and socialized medicine than to come here. It may be that the best tech in the world is here for some specialty, but the administrative and litigious burden offsets most or all of the benefits and the political climate is leaning toward making the situation for you here much worse.Update October 1 2015A commenter challenged my claims that people end up in jail over medical and other debts:I cannot accept your assertion about people being kept in jail or prison until a debt is paid. That is highly unlawful and certainly impractical; how can one ear a living while in prison/jail? And you offer not ONE example or fact to back up your claim. I'm calling BS on this. I know that many are going BK and in terrible financial trouble over medical bills. But going to jail over FTA and being held until medical debt is paid. NO. Collections Payment PlanIf you have received a postcard stating Official Notice Reminder or You Failed to a Make Payment or Appear in Court you must do one of the following to avoid a writ of bodily attachment or collection agency action and driving privilege suspension:They not only jail you for missing a payment, they suspend your driving privileges, making it even harder to keep your job. The commenter is absolutely correct that this is both unlawful and impractical. However, it is posted publicly by many official jurisdictions and reputable news agencies are reporting on harm resulting from execution of these policies.Here is another of those authoritative public postings of the policy:Administrative Order 2014-07 of the Nineteenth CircuitIf a defendant is not current with the required payments and does not appear at the scheduled Collections Court status hearing, the Judge shall order a Writ of Bodily Attachment for the defendant's arrest and delivery to the Indian River County Jail. The Clerk shall prepare and issue a Writ of Bodily Attachment, which shall contain a purge amount, not to exceed $250.00 or the total balance owed, whichever is less, and shall forward it to the Indian River County Sheriffs Office. The state could also lay claim to your estate if you die, prevent you from accessing public housing or revoke your driver's license, she said. Why haven't mortgage rates fallen with interest rates during the coronavirus pandemic?

This is an excellent question. I'll answer it in detail, but fair warning: the answer is somewhat technical.Before the economic effects of COVID-19 began to appear, the Federal Reserve announced that they were cutting the Federal Funds Rate by 50 basis points (.5%). They lower the FF rate to stimulate the economy or raise it to slow excessive economic growth, which would lead to excessive inflation.The Federal Funds rate has nothing to do with mortgage rates. It is the rate banks charge to lend money overnight to other banks. When the Fed lowers the FF rate, the prime rate follows. That will directly affect HELOCs and often credit card rates—but not mortgages.The money you get from a mortgage lender comes from a specialized line of credit called a warehouse line. You don't care where the money came from, because you got the money you applied for at the terms you agreed to.After funding, the lender sells your loan to an investor—Fannie Mae and Freddie Mac are the best-known, although there are many others—for a profit. This is the fundamental business model of the mortgage industry.The investor pools the loans into a type of bond called a mortgage-backed security (MBS). Investors trade them on the open market like other types of securities.When the price of MBS goes up, mortgage rates come down because lenders can get the profit margin they expect with a lower interest rate. If the price of MBS goes up 25 bps (0.25%), the rate of a 30-year fixed-rate mortgage typically goes down about 1/16%.I should have prefaced the above with "in normal times." The situation today is anything but normal. Here is what has changed.When the lender agrees to lock your loan, they are promising to give you specific terms—both the rate and the discount points. One point is 1% of the loan amount. They will fund your loan at these terms regardless of the price the investor pays.If the price of MBS goes down after the lender locks your rate, they will get a lower price for the loan, possibly less than the amount they gave you. To protect themselves against this, they will hedge the loan. The lender "shorts" MBS so that if prices go down, they'll make money on the short position. Readers familiar with trading stock options will recognize this as a "put" or "short position." The profit on the hedge offsets the loss from selling the funded loan at a lower price.If MBS prices go up, however, they lose money on the short position (the hedge), but make more money when they sell your loan. The two cancel each other out. Lenders routinely hedge their pipelines to guard against the normal fluctuations in the market.When rates drop rapidly, as they did following the week of February 24, many borrowers see an opportunity to lower the cost of their mortgage by refinancing. They may have done this even though they have had their loan for a short time. Residential mortgages no longer contain penalties for early payoff, but these early payoffs can cost the lender a great deal of money.Even though the lender who funded your loan has sold it to an investor (Fannie or Freddie, for example), they may retain the servicing: issuing statements and collecting payments. The servicer typically receives a fee of .30% of the loan for doing this.They purchase the rights to service the loan for about 1% of the loan amount, so it will take about three years for them to break even. Loan servicers may increase their servicing income by getting a loan, using the value of their servicing portfolio as collateral.With the rapid drop in rates, borrowers have been refinancing and paying off their old loans far ahead of schedule. The loan servicer, who paid in advance for the rights to service the loan, may not have had that loan long enough to recover the fee they paid.The lender typically earns additional profit from the higher volume of new loans, offsetting the servicing losses. But the COVID-19 crisis has brought much of the U.S. economy to a near-halt. Some borrowers can't complete their transactions because their income has been interrupted or curtailed. The lender has had to pay hedging costs, but do not have the income from selling a funded loan to offset it.Furthermore, the Federal Reserve, in its effort to stimulate a faltering economy, has launched quantitative easing for the fifth time in its history. They enter the market directly to purchase mortgage-backed securities and Treasury Bonds. They have announced that they will buy "unlimited" quantities of MBS, initially up to $50 billion each day.Mortgage bond prices have soared as a result. Daily MBS activity ordinarily looks like this:The green bars represent days where the MBS price closed higher. The size of the bar indicates the amount of change for the day.Here is the activity for the past two weeks:The scale of the two charts is approximately the same. The volatility is dramatically higher.On days with very large green bars (higher MBS prices), lenders will take huge losses on their hedged positions. The broker-dealers issue margin calls to cover these losses. Lenders have to pay tens of millions of dollars to cover them. They must keep their rates higher to recover some of these losses.Although the Federal Reserve's intentions are laudable, their actions have the opposite from the desired effect. The rapid increase in MBS prices because of the Fed's massive purchases is costing lenders millions of dollars in hedging losses. And with many loans not being able to close because of a borrower's loss of income, lenders are not offsetting the hedging losses with additional income.The Fed's failure to communicate effectively—and communication is a critical responsibility--exacerbates the situation. When they executed two massive rate cuts in rapid succession, many borrowers with locked loans broke their locks, expecting better rates elsewhere. The first lender was left only with the hedging losses.There will come a time when the effects of COVID-19 will subside. Lenders will clear their pipelines, and rates will return to normal. Borrowers with locked loans should do their best to meet all conditions to move through the process to funding. If rates improve later, there will always be an opportunity to refinance.For now, we must minimize the health risks associated with COVID-19.
