Universal precautions is as much to protect the patient from us, as it is to protect us from whatever cooties the patient has.

As a doctor or a nurse, what are the craziest examples you have seen of someone making their condition worse?

I am not a nurse or a doctor, but I worked two years as a patient care technician on a orthopedic hall.We had this guy that was a chain smoker and had diabetes. He came in for a knee surgery and after a few days like all knee surgeries he was sent home. He came back a week later because the incision from the surgery was festered and infected. Sometimes diabetes can hinder a wound from healing, so can smoking. After a couple days his incision was no better. So they put a wound vacuum on it. Which basically (and not knowing the medical term) was a large clear adhesive bandage to seal the wound and hold a tube in place that is inserted under the bandage over the wound, and the tube connects to a portable vacuum. Meanwhile as all this is happening he is having friends sneak cigarettes in and he was smoking them in his hospital room. Which was insane, he had to know we would smell the smoke, he just didn’t care. At first his surgeon warned him if he kept it up… 1) The wound wouldn’t likely heal (which it still wasn’t healing) and 2) The hospital and surgeon would have no choice but to discharge him.Now you would think that would be enough but it wasn’t. He convinced a day nurse (that should have known better) to wheel him out to the hospital grounds in the afternoon so he could get some “fresh air” and the nurse was giving him cigarettes. When she got caught, he went back to getting friends to sneak the cigarettes in and smoking them in his room again. After three weeks his wound wasn’t healing and he was still being disrespectful with the no smoking rule. Finally his surgeon gave him a choice… He would have to stop smoking for the wound to heal, or keep smoking and since his knee wouldn’t heal he would eventually wind up loosing his leg from the knee down (I am sure that wasn’t the exact words). Incredibly the guy chose amputation. It was ludicrous. We all had a hard time wrapping our mind around his choice of smoking over loosing part of his leg. He even asked to have it done.That would be the craziest example of someone sabotaging their healing process that I witnessed. What was even worse is he was a RN himself. I don’t know what happened after that. I had my three days off and when I came back he was gone. Positively crazy.
