How do hospitals keep self-harming patients from scratching themselves?

In the Psych Hospital where I worked on Child, Adolescent, and Adult units, a patient’s belongings were gone through very carefully. Sometimes even their liquid hygiene was emptied into another bottle and checked for sharps. Of course the product was poured back into their bottles.All personal items were kept locked up. Pencils only were used on the unit. There were substitute mirrors in their bathrooms. Bathroom doors could be opened quickly from the outside even when they were locked. There were no bathrobe ties, belts, no shoe strings, no bobby pins. no glass allowed. All stuffed animals were searched very well for hidden objects.There were levels of safety patients were put on which determined how closely they were watched. Red level required a staff person to always be no more than one arms length from the patient at risk. When they went to the bathroom the door was left open. A Yellow level required that the patient sleep on his/her mattress in the hall where they could be observed. On admission every patient had a body search done and hair was combed through. No cell phones were allowed unless they had a Doctor’s order and the phones were kept by staff in secure location. Any medication, valuables or money were locked in a safe.We kept Special Observation Logs on each patient every 24 hrs. Patients were checked (eyeballed) every 15 minutes and it was documented were they were, what was their emotional state and what were they doing. These logs were kept faithfully and staff would take turns walking up and down the halls to fill them out every 15 minutes.Sometimes patients would tear up their sheets and try to strangle or hang themselves. In that case they were placed on Red level. A doctor must see the patient every 24 hrs to renew the Red order. Patients ate on styrofoam plates with plastic utensils. If caught trying to cut themselves they were order finger food only and ate in front of staff.There are so many more subtle ways we learned to discover what patient’s were doing to hurt themselves. At times room searches were done for contraband. This meant a thorough search ie, in toilet tanks, running fingers along door sills, checking hems of curtains, viewing under sinks, under mattresses and where ever else there was to search. Mouth checks were done when patient took their meds. We did every thing we could to make things safe for ALL patients. If one patient attacked another he was placed in seclusion until he was ready to talk about it.We couldn’t prevent visitors from bringing in contraband which was most often cigarettes and lighters. In 23 years of working in Psych I was there for three fires lit by patients. Two were severe enough that we had to move all the patients to another floor while the damage was repaired. During that time there were two hangings which I wasn’t there for and one I was. There were two hanging attempts, multiple cuttings, a couple of overdoses. Some patients would cheek their meds, accumulate them and give them to another patient who would overdose on them.I’ve rambled too long. I hope this helps as to how Psych staff keep patients safe.
