I then donned sterile gloves and began to carefully pack the wound with sterile gauze. As I was doing this, I received a sudden sharp slap across my right cheek. I held my hands up to maintain sterility, and was promptly slapped across the left cheek, quite hard.I was stunned. I backed away from the patient, who continued to chat on the telephone, and looked at my attacker, the patient’s wife. In disgust, I left the room and went into the restroom to remove my gloves and gather my wits.I was unable to communicate with the wife, so I indicated that she was to call her daughter, who spoke English. Within moments, the daughter explained that I had utilized both hands to perform the dressing change. This family was Middle Eastern and believed the left hand was “dirty.”I explained that I had sterile gloves on both hands, therefore both hands were equally sterile. The daughter understood, but said her mother was from the old country and would not accept that.As a home care nurse, the primary purpose of our visits were to teach the family to perform as much care as was possible. I asked the daughter if her mother was capable of performing the dressing change using only her right hand, and she indicated that she was. She had seen the dressing changed previously and was confident she could do better than me.I asked the daughter to tell her mother to perform the dressing change and if she were competent, I would sign off on the care.The mother performed without error, and I discharged her husband from home care visits.The primary nurse was furious that I discharged her patient. I told her they all were lucky I hadn’t filed assault charges.Actually, all worked out for the best, as the wife was absolutely competent with her dressing change protocol. It saved Medicare money, and provided the wife with the autonomy she craved.However, did she really have to bitch slap me not once, but twice? Although I felt I had gotten the best of the situation—-particularly while maintaining my professional demeanor , I still had two hand print bruises on both cheeks for several days.
