One was in Memphis during medical school, and the other in residency in Norfolk. One was abused by an uncle, the other by her mother’s boyfriend.My 12-year-old patient in Norfolk presented for medical care for the first time in the last week of her pregnancy. We hurriedly obtained bloodwork and cultures. When she presented in labor two days later, the results of her labs were beginning to come in. She was positive for chlamydia and gonorrhea. And, most distressingly, her rapid HIV test was positive. The rapid HIV test is considered a preliminary test, and although not diagnostic it is correct more than 97% of the time.We debated for a few minutes about the best way to present this information to her. Ultimately, we decided to start off by discussing the gonorrhea and chlamydia and the need to treat it, and then to discuss the additional information about the HIV test.I was honestly expecting screaming and crying as a response. Most 12-year-olds have a very limited and dire understanding of HIV and its prognosis.Her response was mostly a shrug as was her mother’s. Her expression was one of disinterest and unconcern. We confirmed several times that they both understood what we were saying. They agreed to us administering AZT but had no additional questions.In my 16 years in the medical field, I have only once seen a false positive from a rapid HIV test. Thankfully, it was on this patient.
