What are the worst "Oops!" moments in history? A relatively minor mistake that turned out to have disastrous consequences.

Versailles Wedding Hall DisasterOn May 24, 2001, during the wedding of Keren and Asaf Dror, a large portion of the third floor of the four-story building collapsed. 23 people fell to their death through two stories, while another 380 were injured, including the bride who suffered serious pelvic injuries that required surgery. It’s one of the worst building disasters in Israel. This is the video of the incident :InvestigationAn investigation of the event concluded that the event was not caused by a terrorist attack. This was based on the testimony provided by many of the wedding guests present in the building during the disaster. Witnesses reported seeing a dangerous sag in the floor moments before the collapse. An initial inquiry blamed the collapse on the Pal-Kal method of constructing light-weight coffered concrete floor systems which was already banned before the completion of the wedding hall after it was known and proven to be unsafe. Further review pointed to a combination of two alternate causes.Initially, the side of the building that failed was designed to be a two story structure, while the other side was designed to be three stories. Late in the construction process, it was decided that both sides of the building should be equal heights, and a third story was added to the shorter side. However, the live load due to occupancy is typically much greater than the design load for a roof. As a result, the structure supporting the new third story was subjected to much greater loading than was originally anticipated. The effect of this error was somewhat mitigated by the construction of partitions on the floor below, which helped redistribute the excess load well such that no damage was incurred.A few weeks before the collapse, the wedding hall owners decided to remove the partitions. With the load path eliminated, the floor above began to sag several inches. The owners viewed the sagging floor primarily as a cosmetic problem, and attempted to level it with additional grout and fill. However, their approach not only failed to provide additional structural capacity, it also inadvertently introduced a new and significant dead load at the weakened area.During the wedding event in 2001, this significantly overstressed floor section failed, resulting in the catastrophe. The engineer Eli Ron, inventor of the Pal-Kal method of construction, was arrested and subsequently indicted in August 2002 on the charge of manslaughter. Ron had not engaged in any part of the design or construction, but had sold proprietary elements necessary for construction that were installed in a deficient manner.

As a police officer, have you ever gone to a call that made you think “you’ve got to be kidding me”?

I was on routine patrol at about 10 A.M. when I saw a car stopped in the traffic lane, with its emergency blinkers flashing. I pulled in behind it and saw that there was no one in it, so I ran the license plate, prepared to tow the car out of the traffic lane. The dispatcher immediately told me that the vehicle was stolen. That wasn’t something I expected, but I got out of my car and started filling out my paperwork to recover the stolen vehicle. As the dispatcher gave me the information I needed about the vehicle, he asked me, “Do you want to know who stole the car?”I told him that I did. “The report says that the suspect is Bob Sanders, white, male, adult, six foot two inches, 180 pounds, wearing a blue cotton, button-up, long sleeve shirt and tan slacks.”I acknowledged the report and looked up and saw a white, male, adult, about six foot two, 180 pounds, wearing a blue cotton, button-up, long sleeve shirt and tan slacks. He saw me and hesitated, then started to turn down a side-street.I called out, “Mr. Sanders, can you talk to me for a moment?”Sanders’ whole body sagged as he stopped and turned to walk over to me.I thought, you’ve got to be kidding me. This is the easiest felony arrest I’ve ever had and then I placed him under arrest for vehicle theft. The story continued with some other interesting issues, but those are for another question.EDIT: Sorry, I put this in a reply post. I should have added it here.My county engaged in a sort of competition during the holidays in which all the law enforcement agencies would try to arrest the most DUI drivers. This was highly publicized in an effort to reduce DUI incidents altogether. This incident occurred during that time frame. When I returned to the police station with him and read him his Miranda Rights, he agreed to talk to me. (NEVER TALK TO THE POLICE WITHOUT A LAWYER, ONCE YOU’VE BEEN ARRESTED). He explained that the vehicle was his employer’s and he had been sent out to pick up some materials. Instead, he found himself in a bar with some of his friends and they drank and did drugs into the wee hours of the morning. I asked him to perform field sobriety exercises and he failed miserably. I added the charge of DUI, based solely on his statement, well because it was that time of year.Months later, in court for his preliminary hearing, I was on the stand to testify. His attorney, who appeared fairly young approached and asked me (in a condescending tone), “Sgt. Blalock, as a University police officer, how many DUI arrests have you actually made in your career?”“About 300.”This flustered her as she was expecting a significantly lower number. She regrouped and asked, “Sgt. Blalock, my client was on S. 4th Street when you stopped him. Do you usually patrol off the university campus?”The prosecuting attorney actually started laughing out loud, as University police in California are State Law Enforcement officers and have a one mile radius jurisdiction from campus properties. The defense attorney withdrew her question.I learned later that he simply plead guilty to all the charges. He deserved a better attorney.

Is the Coronavirus vaccine causing Bell’s palsy in trials?

News ~ CORONAVIRUS NEWS** Bell's Palsy in COVID-19 Vaccine Trials: Why You Shouldn't Worry **By Anisa Arsenault, Fact checked by Ashley HallDecember 20, 2020, NEWCredit: Verywell / JR BeeAttribution: Bell's Palsy in COVID-19 Vaccine Trials: Why You Shouldn't Worry .Key Takeaways7 cases of Bell's palsy, a type of facial paralysis, were reported by Pfizer and Moderna COVID-19 vaccine trial participants [0f 49,000].The frequency of these cases is no greater than the frequency of Bell's palsy in the general population.Symptoms of Bell's palsy almost always resolve themselves.“””” Reports released from Pfizer and Moderna show that seven COVID-19 vaccine trial participants experienced a type of facial paralysis, called Bell's palsy, in the weeks after vaccination. This rare side effect, in tandem with three reports of severe allergic reactions, has raised some alarm about the safety of the new vaccines. An analysis of the number of Bell's palsy incidents reported so far, however, shows the rate is no higher than the number of people who experience Bell's palsy in the general population. What Is Bell's Palsy? ~ Bell's palsy is a nerve condition that causes partial or complete weakness of one side of the face. The symptoms—like a sagging eyebrow or drooping mouth—come on suddenly, and typically resolve over the course of a few weeks or months. While it's usually difficult to pinpoint a cause, viral infections can trigger Bell's palsy.According to the National Institutes of Health, Bell's palsy affects about 40,000 people in the United States every year, which is 0.01% of Americans. In the Pfizer-BioNTech clinical trial, which included 44,000 participants, 4 people reported experiencing Bell's palsy, which is 0.0091% of participants. The incidents were reported 3,9, 37, and 48 days after vaccination. No cases of Bell's palsy were reported in the placebo group.FDA Authorizes Pfizer As First COVID-19 VaccineIn the Moderna clinical trial, which included 30,400 participants, 3 people reported experiencing Bell's palsy, which is 0.099% of participants. One person was in the placebo group. Among the three not in the placebo group, the Bell's palsy incidents were reported 32, 28, and 22 days after vaccination.FDA Authorizes Moderna COVID-19 VaccineWhile the U.S. Food and Drug Administration (FDA) briefing documents provided by Pfizer and Moderna indicate these rates of Bell's palsy "do not represent a frequency above that expected in the general population," the FDA recommends surveillance for Bell's palsy as the vaccines are distributed among the general population. For anyone who does experience Bell's palsy, the condition is treatable. Oral steroids, especially if administered within 72 hours of symptom onset, are highly effective at restoring nerve function. What This Means For You ~ The few reported incidents of Bell's palsy in COVID-19 vaccine clinical trials should not deter you from getting the vaccine if you are eligible. So far, the number of incidents is no greater than the amount of people who experience Bell's palsy in the general population, suggesting this may be a coincidence.Attribution: Bell's Palsy in COVID-19 Vaccine Trials: Why You Shouldn't Worry .
