Japan, but how Japan is doing so well is currently a million-dollar question no one seems to have the right answer because Japan is not doing much at all. (I will also discuss a well-justified suspicion that Japan is somehow fudging the number).The above is what I just pulled from Google now (12/April/2020). 98 is not a daily figure but total cumulative figures so far. (I think the number just passed 100 today)Japan was not only one of the earliest to have COVID-19 outside of China (6th of January), a large number of Japanese and Chinese travel to and from both countries. Moreover, 1 in 3 of 127million Japanese are over 60, that is +40million. And half of that is over 70s. That +20million people where fatality of COVID-19 is 10%. (8% for the 70s and 15% for 80s and above).Also, the Japanese government’s response is the lamest among developed countries. It shut schools in February but some region in Japan has already reopened school last week because there is so few infected. My old prefecture reopened schools on 7th of April because, as of today, there is only 13 infected in the prefecture and none dead so far.The Japanese government did declare the state of emergency but this only applies to Tokyo area, Osaka area and Fukuoka area. With this declaration, the provincial governments of the declared area can now “instruct” instead of “request” certain business or events to be shut down if there is proper public interest exist. Still no fine or criminal sanction for business ignoring such instruction. Just that such business, aside from bad publicity, maybe opening itself for civil lawsuits if anyone gets hurt from it. (Given it is impossible to pinpoint where and when someone got infected such lawsuits are unlikely to succeed though)K-1 championship, a Japanese MMA event, with +6500 audience, still went ahead on March 22nd, despite the “request” by the provincial government. The reason the Japanese government is so lame is that it does not (and probably cannot afford to) compensate for event closure or business shut down by making such request or instruction legally enforceable.I should also add that Tokyo, Osaka and Fukuoka each have a metro subway system and the first two, in particular, have very extensive ones. Milan, London, Paris, Madrid and New York all have subways and are all having a horrific rate of infection and death. Yet in Japan, these subways still operate. Remote work is encouraged but a large portion of salarymen (and women) still commute (though rush hours is not as bad as before because the companies introduced flex starting time from 7 am to 11 am now). Shops, restaurant and bars are still open though these are suffering as people are trying to stay away from the crowd. Also, Tokyo Disney Land did shut its door partly because Disney can afford to, but most business in Japan can’t, so it won’t.And lastly, Japan hardly does any test as can be seen from the below chart.This is why the government responses from South Korea, Taiwan or Singapore (and somewhat grudgingly China) are praised to heaven in the West but Japan’s response (or lack thereof), is hardly mentioned at all. Almost everyone including a fair portion of the Japanese public thinks Japan is somehow cheating. People in the world are currently desperate for solutions which make sense. Extensive test, shutting shops, office, event and subways, and fining people from breaking social distance rule make sense. No-test, no-lockdown, still commuting, school reopening, shops-open-for-business Japan makes no sense.Only recently, a possibility arose that Japanese may be somewhat uniquely immune to not just COVID-19 but also against other coronaviruses like SARS and MERS as well (which, again Japan inexplicably dodged without much fuss while South Korea was hit hard (2015 MERS outbreak in Republic of Korea). Some of you must have heard recently published study about the efficacy of tuberculous BCG vaccination, in boosting overall immunity including virus and not just TB.Can a century-old TB vaccine steel the immune system against the new coronavirus?Because Japan with dense population used to suffer horrifically from TB, everyone in Japan is vaccinated for TB. Moreover, there is a possibility that a particular strain of BCG vaccine used in Japan called the Tokyo 172-1, may be particularly effective in somewhat boosting the body’s response against coronavirus. Because a Japanese pharmaceutical company used to export Tokyo 172–1 strain of BCG vaccine to East Germany during the cold war, the author did compare the infection of coronavirus between East Germany area and West Germany area, and they did find there is a statistically significant difference in the severity of COVID-19 infection between the East and the West Germany area. Similarly, the reason Portugal which is similar to Spain are doing so well compared to its Iberian neighbour maybe to do with the fact that Portugal maintained BCG vaccination while Spain didn’t. (BUT remember, correlation does not equal causation!).Another possible cause is that Japan having a different and less deadly strain of COVID-19. However, given that Wuhan coronavirus was fairly deadly, and we certainly got it from there, I personally don’t find this convincing but I still mention it here. Anyhow, the reason Japan is doing so well could be due to pure damn luck! But even with some possible immunity boost from BCG and having less deadly strain, Japan is still doing too well.So it is assumed that there must be some number fudging going on. There is no doubt that the reason number of infected is so low in Japan is to do with so little test being performed in Japan. Fewer test, fewer infected on paper. But, while the number of the positive test result will greatly vary by different countries depending on how easy to get the test, death counts shouldn’t differ by the testing regime. This is because anyone who is hospitalised due to symptom of pneumonia, especially difficulty breathing will be presumed to be COVID-19 positive, and they will get X-ray, CT scan and COVID-19 test without exception. (Doctors and nurse are understandably afraid because the large viral dosage of COVID-19 could prove fatal to them). Therefore, death of COVID-19 positive patient at the hospital will be hard to conceal unless the entire population of doctors and nurse from different hospitals in Japan are somehow in on a giant conspiracy to deceive Japan and the rest of the world. The only margin of error is death outside of hospitals but most countries do not count these deaths in COVID-19 death counts anyway.So, there is some consensus that the Japanese low number is probably genuine. Maybe not as good as the official number but still good nonetheless. (+100K people die from pneumonia in Japan every year so some of these must hide really old infected persons who didn’t manage to reach a hospital before passing away, but this still doesn’t change the fact that coronavirus death in Japan is so low that it can hide behind overall pneumonia death)So a big question is why so little test. It is not due to the supply of the test kits. Japan uses just a fraction of its coronavirus testing capacity according to Reuter. And I just stumbled upon a Facebook post of a doctor who is currently a committee member for coronavirus committee by the Japanese Ministry of Health.おのクリニック (a clinic copy/pasted the article from the Facebook)The reason Japan doesn't use test appear to be that, as far as Japan is concerned, there is no point to it.Firstly, there is a little clinical benefit for testing because there is no known cure specific to COVID-19. There is no unique cure doctor can try just because you test positive. Only treatment one can receive is what people suffering from pneumonia receive. And if you get hospitalised for pneumonia symptom (especially difficulty breathing), you are presumed to be COVID-19 positive anyway. Also, It doesn’t matter you do get tested and receive negative result because PCR testing is known for a high rate of a false negative. For mild symptom, the false positive rate is as high as 30%, i.e. 30% of people who actually have it get test result saying they don’t have it. This will improve to 10% or less range for someone with strong symptoms (and very clear X ray and CT result indicating pneumonia) but 10% is still too high. You still have to put this person to isolation otherwise the patient could infect others. Because of this, even if you get a negative result, if you show symptoms, you are still assumed to be COVID-19 positive. So all doctors need is a symptom of pneumonia, XRay and CT. and patient get pneumonia symptom treatment including oxygen support (remember, there is no cure so these can only assist your body’s natural immunity fighting the illness). And guess what? Japan has the highest number of CT machine per capita in the world. Testing for COVID19 doesn’t significantly add to a clinical decision (except the expectation that an infected person’s symptom might deteriorate quicker). But Japan is best equipped to test where it count, which is test for pneumonia.The real benefit of testing is not clinical but for preventing spread of infection by identifying and isolating infected, and by extension, contact tracing, where the COVID-19 positive person’s past contact is traced and then anyone who came in contact with that person is tested and if positive, isolated. South Korea’s use of the mobile phone to trace infected’s past movement, then publishing the locations online so anyone who suspect being in vicinity can get tested is very famous now. And remember what WHO’s Director-General’s said 'We have a simple message for all countries: test, test, test. Test every suspected case.”His statement continues, “If they test positive, isolate them and find out who they have been in close contact with up to 2 days before they developed symptoms, and test those people too.”However, the transcript of his speech posted in WHO added something else after that.WHO Director-General's opening remarks at the media briefing on COVID-19 - 16 March 2020That additional NOTE is important. Without symptom, the false-negative rate is too high. In fact, even with a mild symptom, the false-negative rate is still as high as 30%. With no symptom, false negative (test says you don’t have COVID but you do) and, to much lesser degree, false positive rate (test say you have COVID but you actually don’t) are so high that it is useless and misleading. So “suspected” here doesn’t mean someone who just had contact with infected or happen to be where infected visited before the place got disinfected. It must mean “had contact and have symptom”.And in Japan, the test is offered if (1) you have a symptom of pneumonia (especially breathing difficulty) or (2) at-risk person (old age, diabetic, heart or lung disease, cancer, etc) with persistent mild symptoms like cough or fever And even with this tighter criteria, we still getting only 7% positive test result. In the state of emergency area, this has gone up to 10%, meaning despite showing symptoms, +90% get negative results. Even if one consider false negative, only a very small portion of people who show symptoms genuinely have COVID-19.And in Italy, when the outbreak happened, they went for massive indiscriminate testing. They even tested the whole adult population of the town which was the epicentre of the outbreak. This is why Italy’s test figure is sky-high.And in Japan, it is understood that that led to the collapse of medical capacity, because people with mild symptoms (most of whom will recover on its own) gatecrashed hospitals enmass, and since the only way to get capacity limited medical treatment is to be showing more severe symptom than the other persons, people start to inflate their symptom by coughing more excessively in front of doctors, not only because they intend to deceive but because of the phycological effect of thinking you could have a deadly disease. Somehow, that itchy throat you feel feel ITCHIER! To make it worse, majority with symptoms including fever are due to something else other than COVID-19 but because the accuracy of the test is low, the negative test won’t clear the risk, so that person has to be treated as positive (even though, in reality, that is not true). Hospitals very quickly run out of isolation beds. And here, the South Korea genuinely excelled by providing isolation accommodations for people with low and mild symptoms, hence relieving pressure from hospitals AND preventing secondly infection at home with immediate family members.Moreover, false negative (test say you don’t but you actually do) is so high that possibly 3 in 10 infected person get a negative result. If that happens, those 3 may not obey the instruction to self-isolate for two weeks, hence spreading the infection possibly as a super spreader due to riskier behaviours especially if that person is young. As far as what I’m informed, Italy eventually gave up on extensive testing. And also, while it is widely reported that South Korea deployed extensive testing and that is the secret of their success, later, from what I read in Japanese media, they actually did raised the hurdle of getting the test to the textbook standard for testing during a pandemic. And that standard says.“TEST ONLY THOSE WHO ARE SHOWING SYMPTOMS FOR DAYS OR MORE”This is why WHO itself had to add NOTE in the transcript. You diverge from textbook instruction and your medical capacity will collapse because the hospitals will be flooded by people with a mild symptom, many of whom may not be infected but will be after congregating with a small number of infected while seeking to be tested. Lockdown and social distancing will.And lastly, here is what is specific to Japan. Japan still do contact trace people who tested positive because we test so few and even fewer show positive results. And so far, we found that COVID-19 is LESS, not more infectious than Influenza. Usually, when one contact trace influenza, it is common to find that all or most immediate family, all friends in close physical contact (such as boy/girlfriends) and some of classmates, office colleague at work or school are infected. In the case of COVID-19 in Japan, so far, in most case, only immediate family who lives with the infected and lovers (boy/girlfriends) who had high physical contact were infected but very few classmate and workmates are. If we find someone infected, in most case, we just need to isolate the infected’s immediate family or boyfriend/girlfriend. While we still do contact tracing, there is very little gain from it, because additional infected outside of immediate vicinity is so few. This is why, AS FAR AS JAPAN IS CONCERNED, there is very little point of testing from contact tracing point of view.How could this be? Some of you might have seen the below. COVID-19 is supposed to be nearly twice more infectious than influenza.And 2.5 is a very high number because, as can be seen from the below. In 30 days (TAGE means days in German), 406 people could be infected, while for 1.25 (near influenza figure) only 15 people are infected.With such high R0 figure at above 2, infections, and subsequently, death will grow exponentially unless countered by very aggressive suppression measure like lockdown. In Japan where only mild mostly voluntary social/economic restrictions are in place, COVID-19 is currently actually less infectious than influenza, and this is why the growth of death is basically linear in Japan because R0 is just about slightly above 1. Also, if your hand touch infected surface like plastic, then, in turn, your infected hand touching your mouth, nose and eyes are a sure way to get infected. This is why wearing a mask outside, even a bandana, over your mouth and nose could be effective because it will prevent you from unconsciously touching your nose or mouth with your hand. So if you sanitise your hand immediately after coming out from a supermarket, or wash your hand immediately after you come back home, and only then remove your bandana, (then wash your bandana before the next outing), you greatly enhance your chance of avoiding physical infection.Also, we are now familiar with 2meter distancing rule. Because if someone talk or cough, then water droplet would spread with a radius of 1–2m but they will quickly drop to the ground and become non-infectious (unless it drops to plastic or metal surface, in which case, it will remain infectious for few days till someone touches it).So what about aerosol. But this is due to two things. Japanese subway used to be packed. Train company staffs used to push people into the carriage. That will straight away cause infection through water droplet because people are up in their face to each other. This is no longer the case in most times because the companies in Japan now introduced flex-time to spread out rush hours and encouraging people to remote-work. Secondly, this is Japan-specific but in Japan, people even before COVID-19 wear a mask as a matter of social etiquette to prevent spreading flu and cold. And thirdly, in Japan, it is considered rude to talk on train or bus, including talking to mobile phone.
