I keep hoping that the photos don't amount to anything important.The exam starts, the questions are easy, I breeze through chemistry and physics, slow down some at math (so many integration problems, what?).Two hours have passed. The invigilator starts calling students to the desk to complete attendance and registration.Moment of truth.... My name is called and the invigilator asks me for a photo. I'm at a loss. I explain to him my blunder and he asks me to see him after the exam.After the exam, I'm handed a paper to write a decleration letter to state that ‘I had inadequate documents required for the exam’ and hence my exam is void of evaluation.Words cannot express how stupid it felt. If you have moved residence, for example, you should update your voter ID card so it reflects your current address. Your name has to be added to the electoral roll in the constituency where you now reside, and it has to be removed from the roll in the constituency where you lived previously.The Election Commission of India has simplified the process to update your address in your voter ID card. Instead of making different applications to include your name, to change the address on your voter ID card and to remove your name from your old constituency, you can now do all of this through a single application form.How to Transfer my Address in Voter ID cardTransferring your address in your voter ID card can be done by submitting an application form as well as proof of your current address to the Election Officer in your constituency.Known as transposing of name from electoral roll, you name will be moved from your old constituency’s electoral roll to the new one, retaining all your details. This is my experience getting a disability certificate for physical disabilities in Bangalore:Identify the government hospital that caters to your region. If you are not sure which hospital caters to your region, try calling the enquiry number of one of the following hospitals for Bangalore:BOWRING LADY CURZONHSIS GOSHIYAINDIRANAGAR GENERAL HOSPITALJAYANAGAR GENERAL HOSPITALKC GENERAL HOSPITALVANIVILAS HOSPITALVICTORIA HOSPITALESIC Model Hospital Rajajinagar (they do disability certificates only for industrial accidents etc.)The complete list for Karnataka is here:https://www.karnataka.gov.in/hfw/decell/Documents/District%20Hospital%20list.pdfOnce you have identified the hospital, make a visit to the hospital without the person with disability and enquire about the procedure there. For the most part, it is figuring out the department and the person who is incharge of the process. Once you identify the person, check on the next camp date when the disability assessment will be done. Also ensure you have collected all the documents required. In our case it was Aadhar card, previous medical reports, a photograph of the disabled person, clearly showing the disability. You can check with the photo studio near the hospital to know what kind of photo needs to be taken. It is a passport size photo - ensure you get at least 6 copies - since we finished 5 of them for the full process.Next day, go to the hospital with the person with disability, along with aadhar card, photo copy of aadhar card, photographs taken earlier showing the disability. Contact the clerk you identified during your previous visit, who is incharge for the certification process. Collect a form to register for the disability certificate, fill it out with the details and get it signed by the person with disability. Affix the photos and submit the form. You will be asked to make a photo copy of the application form for their reference.Get information on the date when the next disability assessment camp is scheduled and mark your calendar.Keep the application form / xerox whichever they give you safely.On the day of the camp, go to the hospital with all all medical reports, the application form, aadhar card, photo copy of aadhar card, photographs taken earlier showing the disability. On that day, they will give you a book (yellow colour) where you have to enter details of the person with disability, affix a photo and ask the person to sign. This will be his/her disability ID card. Then the doctor assessment will happen and the will be made both in the ID card and the application form with details and % of disability. Once the assessment is done and entries made, you will be asked to come once more on a specific date to collect the signed and sealed disability ID card and certificate. (The registration form that you filled out in point no. 3 above will become the disability certificate.) You will be asked to make a photo copy of the application form for your reference. This should be brought back on the day you collect the signed and sealed disability ID card and form.On the day when you are asked to collect the disability certificate and ID card, go to the hospital (the person with disability is not required for this procedure) and contact the same clerk who will hand over the form to you. You will have to carry the xerox copy of the application that you took on the camp day. Also, carry the photos and aadhar card and aadhar card photo copy. You will have to give a copy of the aadhar card and the xerox of the registration form for you to collect the disability ID card and certificate. You will be instructed to go to NIMHANS and get another seal and signature. (If not, double check if it is required. Or else just ask if the certificate they are issuing is good to get disabled bus pass / railway concession etc. Then they will tell you that you have to go to NIMHANS and get the seal and signature.)On the same day you collected the disability certificate and ID from the government hospital for your area, go to the District Disability Officer’s office, which is NOT in NIMHANS but a small gated lane just after KIDWAI exit gate. (It is easy to miss it since autos / traffic / hawkers will be hiding the view to this road.) Go into this road and keep following it until you see a building on the right. (It is a small one floor building.) Walk into this building and request the person there for the seal and signature on the Disability ID. They will ask you to make a full photo copy of the disability id card book from cover page to last page, and also a photo of the person with disability. Hand them over the disability ID card book, the photo copy of the id card book, copy of the aadhar card, and a photograph of the person with disability. They will ask you to come after 1 or 2 days and collect the signed and sealed disability ID card. On that day again, they will ask for a photograph of the person with disability.On the last day go back to the District Disability Officer’s office and show them the aadhar card of the person with disability and the disability certificate (that they did NOT collect from you in point no. 6). They will ask for the person’s photo. Once you provide them the photo, they will give you the disability ID card with the seal and signature of the district disability officer.That brings you to the end of the process of procuring a disability certificate. The next steps like getting Nirmaya card etc. may be pursued after this.Good luck and let me know if I have missed something or if there are other experiences.
