Yes, One has to fill up a separate DS-160 form for prime applicant and any of its H4 dependent (like spouse or child) for US Visa interview. Few steps are below based on my experience.(1) Once you submit DS-160 form, you will have separate DS-160 confirmed number for each applicant.(2) Go to https://cgifederal.secure.force.com/?language=English&country=India for interview in India and create your profile, if not done already.(3) Once logged in, click on New Application/Schedule appointment to book a new appointment. Fill up all the information as required.(4) In Step # 5 it will prompt you to fill up prime applicant(your’s) passport and Petition (for H1Bs) etc details. On this same page, prime applicant attaches his/her DS-160 number.(5) In Step # 6 , you can fill up your H4 dependent info and tag their DS-160 numbers.(6) In the next steps, when you make visa fee payment; you will be given a receipt number (same for all applicants in such cases). you can tag that receipt number against all applicants in that appointment on this page.(7) Finally, you proceed to appointment calendar page where whatever slot you pick up, is applicable for all applicants. As a confirmation, you can see all names when you finally print the appointment confirmation.So that’s how whole family appears for interview at the same time slot. Hope this helps!
