What was the grossest thing you witnessed while working in healthcare?

I’ve seen a lot. Generally it comes with the job.But 2 patients stick with me.One patient had just arrived from a nursing home. If I remember, she had fever and changed mental status. I was doing her intake physical and found a 4x4 gauze taped over her sacral area. I took it off and more gauze was stuffed into the hole under the first gauze. I unpacked the wound but for some reason, some of the gauze was stuck in the wound. I flushed saline into the wound, but the gauze would not release. Then I realized. The remaining gauze was HOOKED ONTO THE BONY PROCESS OF HER VERTEBRA! The smell of rotting flesh was unmistakable. It made my aide with 25 years experience retch.Obviously, the nursing home never notice the wound till they moved her for transport. Then they tried to cover their tracks. I would have rather been told that they had just discovered it upon transport, but they did not mention it in report at all.Second patient had Steven Johnson syndrome which is caused by various things but usually a reaction to an antibiotic. Her skin literally was peeling from her body. She was incontinent. We had to turn her to clean her. If you touched her, her skin would just fall away. We would wrap her in a sheet when we had to touch her.. When we would unwrap her, fresh wounds would be present and skin would collect in the sheet. We could not keep her sheets clean without hurting her further. We ended up collecting the skin off the sheet after we had finished.Poor things.I’ll never forget those two.

How do I avoid leaky diapers at night?

My kids were super heavy pee-ers, especially at night, and especially because the last two night nursed until 20 months each, and so were not only well-hydrated going into the night, but super hydrated throughout it. Pee-outs were way more frequent than blowout poops in my house and bed (I coslept). Because I coslept, I had even more buy-in to solving this problem (changing a wet baby who leaked AND changing myself AND my sheets at night is a lot of work when you’re sleep-deprived!).Here are some things that worked for me:First and foremost, investing in nighttime diapers was huge. Truly, truly huge. I used Huggies® OverNites and was very, very grateful for them. This went the furthest in solving the issue for me. (These are also great for long car rides or airplane flights, btw.)An alternative to OverNites would be sizing up a diaper size, which just gives you more surface area (and gel underneath) to fill. One downside of this though is that without a nicely fitting seal around the legs or top, you still can get a leak if your child (especially boys, who, *ahem*, may be pointed in bad directions) pees a lot while side sleeping, etc.Position the penis, if you have a boy. Seriously. Where that thing is aiming is super important. Trial and error is good, but generally, having it aimed at a leg hole is bad. Where that pee goes is important. You want to aim it towards the more absorbent areas (and obviously also the baby’s comfort, this is not the time to do weird contortion tricks).Work on limiting the fluids right before bed-time and/or in the middle of the night. This is trickier, of course, because you also want to keep children/babies hydrated, but you’ll likely need to do this anyway as you move into the potty training stage, so it doesn’t hurt to build up a habit of the bulk of fluid intake being in the AMs, lunchtime, and throughout the afternoon/playtime. An earlier dinner time is fine for full fluid intake, but if you eat in the hour or so before bed, limiting fluid at night is probably good too. Alongside that, limiting the salty food intake before bed helps too, because then children are less thirsty at bedtime/middle of the nights (and anyway probably healthier overall).Ninja-change it. Seriously. If you wake up in the middle of the night and it’s getting full, change it. You’ve got to get good at ninja-changing anyway, because it’s an important life skill. Change the diaper at the very last second before the child goes to sleep, and figure out a time in the middle of the night where it makes sense to change it again. If they’re sleeping through the night, maybe it’s when you (or another caregiver, whoever’s up latest!) goes to bed. If they wake up, just pick a point midway through maybe.Good luck! I won’t lie, during growth spurts or heavy nursing times, even with OverNites, I wasn’t above sleeping with the baby on a towel ‘just in case’. You do what you have to in order to survive the young, sleep-deprived years.
