It must be on their website and available to you upon request. It describes how they implement the access rights law.

Ask for the original provider's notice of privacy practices. If they are covered under HIPAA they are required to have one. In that document you must be given an option to request they amend or correct your records.However, they do have options to deny the request based on certain circumstances.

As someone who worked for Blockbuster in Texas, my memories are…different from those of many customers. Management’s response when informed? Reprimanded me for ringing up his videos wrong, and left the supervisor in charge.When my 3 month trial period at Blockbuster ended, they offered to promote me. I gave my two week’s notice instead.As someone who actually worked at Blockbuster in Texas, I can say with absolute sincerity that I am glad to see their final store closing: this could not have happened to a better company. Literally.
